摘要
目的分析衢州地区1 280例宫颈病变女性中人乳头瘤病毒(human papillomavirus,HPV)21种基因型的分子流行病学特点。方法对衢州地区1 280例宫颈病变的女性进行21种HPV基因型分型检测,比较不同基因型的流行病学特点。结果本组病例中总HPV感染率为55.31%(708/1280),其中HR-HPV感染率为38.83%,LR-HPV感染率为16.48%,高危型最常见类型为HPV 16、58、52,低危型最常见类型为HPV6、11。HR-HPV亚型中16、58、52是主要的独立影响因素,而LR-HPV亚型中6、11是主要的影响因素。HPV单一型感染率11.1%,两种亚型合并感染率42.9%,三种亚型合并感染率30.6%,四种及以上亚型合并感染率15.4%。不同年龄段感染HPV及感染HR-HPV的比例均有显著性差异,最高为31~40岁组,分别占67.39%和50.58%,其次为21~30岁组,分别占61.75%和45.78%,HPV感染与宫颈病变程度呈正相关。结论本研究提供了衢州地区宫颈病变女性的21种HPV基因型分子流行病学资料,为本地区宫颈癌的临床防治工作提供了可靠的科学依据,对本地区今后疫苗的开发、应用也具有重要价值。
Objective To study the molecular epidemiological characteristics of 21 genotypes of human papillomavirus (HPV) among 1 280 Cases of cervix lesions women in Quzhou. Methods The 21 HPV types were determined among 1 280 women who were suffering from cervix lesion in Quzhou. The molecular epidemiological characteristics of 21 HPV types were compared. Results The total positive rate of HPV was 55.31%(708/1280), high-risk form of which accounted for 38.83%, low-risk form of which was 16.48%. HPV16, 58 and 52 were commonly seen in high-risk form and HPV6 and 11 were mostly seen in low-risk form. HR-HPV subtypes 16, 58, 52 were the main independent factors, and the LR-HPV subtype 6, 11 were the main influence factors. HPV single infection rate was l 1.1%, two subtypes associated with an infec- tion rate was 42.9%, three subtypes associated with an infection rate was 30.64%, or more subtypes associated with an in- fection rate was 15.4%. The positive rates of HPV and high-risk form of the age group of 31-40 years were the highest (67.39% and 50.58%), followed by the age group of 21-30 years (61.75% and 45.78%). HPV infection rate was positively related to the degree of cervical lesions. Conclusion This research offers molecular epidemiological characteristics of 21 genotypes of HPV among 1 280 Cases of cervix lesions women,and provides the reliable and scientific evidence fnr preven- tion and treatment of cervical cancer in Quzhou. It is of great value to further develop and apply HPV vaccine in this region.
