宫腔镜术后球囊导尿管联合透明质酸钠综合治疗重度宫腔粘连的疗效分析

Efficacy of integrated treatment with balloon urethral catheter and sodium hyaluronate in intrauterine re-adhesions preventive after hysteroscopic lysis of severe intrauterine adhesions

目的探讨宫腔粘连宫腔镜分离术后用宫腔放置球囊+宫腔内注入透明质酸钠联合宫内节育器(IUD)的综合方法对重度宫腔粘连的治疗效果。方法将62例重度宫腔粘连患者行宫腔镜下粘连分离术,术毕随机行不同处理,30例采用IUD治疗,32例采用IUD和宫腔放置球囊+宫腔内注入透明质酸钠综合方法治疗。两组均于术后行抗生素预防感染和3个周期的人工周期治疗。术后随访3个月,记录月经改善及宫腔恢复情况。结果综合疗法对改善术后月经及预防再次粘连疗效比IUD好,差异有统计学意义(P<0.05)。结论宫腔镜宫腔粘连分离术后应用综合疗法对预防再次宫腔粘连效果明显优于IUD疗法。

Objective To estimate the efficacy of the comprehensive treatment catheter and intrauterine injection of sodium hyaluronate joint intrauterine device placed with intrauterine balloon urethral （IUD） after hysteroscopic lysis of severe intrauterine adhesions. Methods Hysteroscopy confirmed 62 cases of severe intrauterine adhesions underwent hysteroscopic adhesion dissection. Randomly to different treatment, 30 cases of treatment with IUD, while 32 cases of comprehensive treatment with placing balloon urethral catheter and injecting sodium hyaluronate＋placing IUD. Both groups were used of antibiotics to prevent infection and three artificial cycles. Both groups were followed up for three months, recording men- strual improvement and intrauterine recovery. Results The complex treatment was more effective than the IUD to improve the postoperative menstruation and prevent intrauterine re-adhesions, the difference was statistically significant （P 〈 0.05）. Conclusion The integrated approach to intrauterine re-adhesions preventive after hysteroscopic lysis of severe intrauterine adhesions was better than that of the IUD therapy.
