乳腺癌根治术后皮下积液和皮瓣坏死的预防处理

Preventative treatment of subcutaneous fluid and skin flap necrosis complicating radical mastectomy

目的探讨乳腺癌根治术后并发皮下积液和皮瓣坏死的原因及预防处理方法。方法对2000～2011年115例乳腺癌根治手术病例进行了回顾性分析。结果 115例乳腺癌术后并发症发生率为16.5%,其中皮下积液10例,占8.7%,皮瓣坏死9例,占7.8%。经过穿刺引流、创面换药,均在5～20 d后治愈。结论通过改进细化手术操作、合理有效多管引流、适度加压包扎等一系列措施,可减少皮下积液与皮瓣坏死的发生。

Objective To investigate the causes and preventative treatment methods for subcutaneous fluid and skin flap necrosis complicating radical mastectomy. Methods One hundred and fifteen cases of radical mastectomy from 2000 to 2010 were analyzed retrospectively. Results The incidence rate of complications of the 115 cases of radical masteetomy was 16.5%, including 10 cases （8.7%） of subcutaneous fluid and 9 cases （7.8%） of skin flap necrosis. Through puncture drainage and wound dressing, all the patients recovered after 5 to 20 days. Conclusion Through the improvement of surgical operations, rational and effective multi-tube drainage, appropriate pressure dressing and a series of other measures, the occurrence of subcutaneous fluid and skin flap necrosis can be reduced.
