摘要 目的探讨抗环瓜氨酸肽抗体(anti-ccp)诊断风湿性关节炎的临床意义。方法用散射速率法和酶联法检测类风湿性关节炎(RA)患者组95例和非RA类风湿疾病患者组140例外周血清类风湿因子(RF)和anticcp。结果 RA患者组中,anti-ccp阳性率为78.9%(75/95),RF阳性率67.4%(64/95);18例早期RA患者(2~6月)16例anti-ccp阳性(88.9%),而RF阳性率仅为61.1%,64例RF阳性RA患者中59例anti-ccp阳性,而31例RF阴性患者中16例anti-ccp阳性。RA患者组anti-ccp和RF灵敏度分别为78.9%、67.4%,特异性分别为84.3%、95.7%,AUC分别为0.775、0.852。结论 anti-ccp对诊断RA具有较高的灵敏度和特异性,且较RF能早期诊断RA。 Objective To investigate the clinical significance of CCP diagnosing rheumatoid arthritis. Methods The RF and anti-ccp of 95 RA patients and 140 non-RA patients in peripheral blood were detected with rate nephelometry and ELISA. Results The anti-ccp resuhs of 75 samples of RA were positive , the positive rate was 78.9% (75/95), howerer, the positive rate of RF was 67.4% (64/95) . The anti-cep results of 16 samples were positive ( 88. 9% ) in 18 patients with early (2-6 months) , the positive rate of RF was only 61.1%. 59 samples of anti-ccp were positive in 64 samples of RA patients that the RF results were positive , however, 16 samples of anti-ccp were positive in 31 patients that the RF results were nega- tive. The sensitivity of anti-ccp and RF were separately 78.9% and 67.4% in RA patients. The specificity of anti-ccp and RF were separately 84. 3% and 95.7%. The AUC were separately 0. 775 and 0. 852. Conchrsion The anti-ccp had higher sensitivity and specificity than RF. The anti-ccp in diagnosing RA could be earlier than RF.
