库存血液胆碱酯酶水平变化

A Preliminary Report on the Alteration of Cholinesterase Level in Banked Blood

目的 观察库血保存天数与血浆胆碱酯酶 (CHE)水平变化关系 ;探讨急性重症有机磷农药中毒 (AOPP)抢救中合理选择用于换血疗法或者输入的血源。方法 输血前随机留取保留不同天数的库血测定血浆 CHE值 ;血液中心采血当日留取正常人血浆、即日测定 CHE值作对照组。 CHE测定用 BM公司的胆碱酯酶试剂盒 ,Boch公司的Cobas- Fara 自动分析仪测定。结果 库血保存天数 6～ 1 1、1 2～ 1 7、1 8～ 2 1三组血浆 CHE值与正常对照组比较 ,均显著低于对照组 (P <0 .0 5 ) ,下降百分比分别为 75 %、77%、82 %。保存天数与 CHE值呈负相关 (r =- 0 .42 85 ,P <0 .0 1 )。结论 随采血后库血保存天数的增加血浆 CHE值逐渐下降。 6 d后各组与正常组比较均有显著差异 ,使用该期间库血、对补充胆碱酯酶而言价值不大。本报告为强调输入新鲜血液 ,保证胆碱酯酶输入 ,避免浪费血源 。

Objective[WT10.5,10.25BZ] To stydy the relationship between the preservation days of banked blood and the alteration of plasma cholinesterase level with the aim of making proper selection of banked blood in emergent treatment of acute organophosphorus pesticide poisoning(AOPP0).Methods The banked blood that had been stored for different length of time before transfusion was selected to determine the plasma cholinesterase levels. The normal plasma cholinesterase value had been determined in Shanghai Bood Center serving as control.The cholinesterase value was determined by the kit from BM Company and the Cobas Fara II automatic snalysor of Roch Company.Results As compared with the normal control group,the plasma cholinesterase values of the banked blood on the sixth day,twelveth day,and eighteenth day were all significantly lower than thatof the control group (P<0.05).They decreased 75%,77% and 82% respectively,the lengyh of preservation time being in negative correlation with the cholinesterase value (r=0.4285,P<0.01).Conclusion It is proved that the plasma cholinesterase level of the banked blood.decreases during preservation.It is of no value to use the banked blood stored over six days in the case of first aid treatment of AOPP.In such emergent case,it should be emphasized to transfuse fresh blood to ensure the transfusion of highly active cholinesterase.
