质子泵抑制剂对老年溃疡病人胃内酸度的影响

EFFECT OF PROTON PUMP INHIBITOR ON INTRAGASTRIC ACIDITY IN ELDERLY PATIENTS WITH PEPTIC ULCER

观察质子泵抑制剂对老年消化性溃疡病人胃内酸度的影响。对经内镜证实的老年溃疡病 2 0例。非老年溃疡病 1 2例 ,测定其 2 4h胃内 p H节律。分别测定治疗前及服质子泵抑制剂 7天时 2 4h胃内 p H动态变化。结果 :老年溃疡病人服药前 ,2 4h p H>4的总时间百分比、中位 p H、平均 p H,分别为 0 .0 6 3、1 .34、1 .92 ;服质子泵抑制剂 7天时 2 4h p H>4的总时间百分比、中位 p H、平均 p H分别为 0 .82、6 .6 8、6 .45 ;与治疗前相比 ,各项指标差异均有显著性意义 (P<0 .0 1 )。非老年病人与老年病人相比 ,用药前后胃内各项 p H指标无明显差异 (P>0 .0 5 )。结论 :老年消化性溃疡病人服用质子泵抑制剂前后 2 4h胃 p H动态变化与非老年溃疡病人大体相同。质子泵抑制剂通过明显增高溃疡病人中位 p H值和 p H>4百分比时间 。

To observe the effect of proton pump inhibitor on gastric acidity in elderly patients with peptic ulcer, the circadian pattern of 24 hour intragastric pH was analyzed in 32 patients with peptic ulcer before and after medication by 24 hour pH monitoring. Before medication, percentage of total time with pH>4 during 24 hour median pH and mean pH in elderly group were 0.063, 1.34 and 1.92, respectively. Seven days after the treatment with proton pump inhibitor, percentage of total time with pH>4, median pH and mean pH were 0.882, 6.68 and 6.45, respectively. The differences were highly significant ( P <0.01). Compared elderly patients with non elderly patients, no significant difference was found in pH parameters before and after medication.The values of percentage of total time with pH<4 and 24 hour median pH were simiar between elderly and non elderly groups. Proton pump imhibitor is effieaciour in the treatment of peptic ulcer through raising the level of intragastric pH.
