超声对十二指肠乳头癌的诊断价值

Diagnotic value of ultrasound in identifying carcinoma of duodenal papilla

目的探讨超声对十二指肠乳头癌的诊断价值。方法回顾性分析47例十二指肠乳头癌患者的超声影像资料,并与CT及胃镜结果对照。结果 47例十二指肠乳头癌,超声表现为结节肿块型36例,局限管壁增厚型3例。超声检查47例,术前准确诊断十二指肠乳头癌5例;CT检查35例,术前准确诊断14例;胃镜检查24例,术前准确诊断15例;超声、CT、胃镜诊断符合率分别是10.6%、40%、62.5%。超声、CT、胃镜联合检查19例,诊断十二指肠乳头癌17例,诊断符合率为89.5%,与上述单一诊断法的差异有统计学意义(P<0.01)。结论超声能显示肝内外胆管扩张情况及十二指肠乳头区肿块,能为十二指肠乳头癌术前诊断提供重要的信息。

Objective To investigate the diagnostic value of ultrasound in identifying carcinoma of duodenalpapilla. Methods Ultrasound data of 47 patients with carcinoma of duodenal papilla were retrospectively analyzed and compared with CT and gastroscope findings. Results Under ultrasound, masses in the duodenal papilla region were reported in 36 cases and thickening of duodenum wall were reported in 3 cases. Carcinoma of duodenal papilla were detected before surgery in 5 out of 47 patients who underwent ultrasound, in 14 out of 35 patients who underwent CT, and in 15 out of 24 patients who underwent gastroseope. The diagnostic rates of ultrasound, CT and gastroscope for carcinoma of duodenal papilla were 10.6%, 40% and 62.5% respectively. The combination of ultrasound, CT and gastroscope for 19 patients found carcinoma of duodenal papilla in 17 patients. The diagnostic rate of the combination for carcinoma of duodenal papilla was 89.5%. Conclusion Ultrasound can show the dilatation of intra-/extra-biliary ducts and mass in the duodenal papilla region. This finding can provides important information for the diagnosis of carcinoma of duodenal papilla before surgery.
