新型机械臂在脑外科定向手术中的应用 被引量:17

摘要 目的 临床应用脑外科机械臂系统 (国产CRAS HB型 )辅助完成脑立体定向手术。方法 利用机械臂系统的计算机图像定位软件、穿刺径路设计模拟引导装置、智能机械臂平台和放射性核素注入控制模块等功能部件 ,成功辅助完成脑立体定向手术 32例 ;其中脑肿瘤内放疗手术 2 3例、脑深部病变活检 3例、脑脓肿抽吸 2例、脑内血肿排空 2例、金属异物摘除 2例。结果 本组患者手术靶点定位准确 ,均一次操作成功 ,无手术并发症。除 3例活检阳性外 ,2 9例术后随访 6~ 2 4月 ,手术有效率 86 %。结论 该机械臂系统设计结构合理 ,操作集成实用性强、定位精确且操作方便 ,具有较为广阔的应用前景。 Objective TO set up a robot system (domestic CRAS HB type) in assistance of stereotactic operations. Methods The robot system included the guiding software of computer images, the hardware of machine arm and the control injection of radioisotopes. 32 cases of stereotactic operations were successfully performed under local anesthesia with this robot. The operations contained the intratumoral irradiation in 23 cases, biopsy of deep brain lesion in 3 cases, aspiration of abscess in 2 cases, evacuation of hematoma in 2 cases and remove of metal foreign bodies in 2 cases. Results Among all cases the targets were arrived with high accuracy. There was no serious complications in this group. Besides 3 cases of positive biopsy, 29 cases were followed up 6~24 months; the effective rate of operation was 86%. Conclusions The design of the robot was rational; the operative integration was practical; the directive target was accurate and the robot was easily used. The robot has a wide clinic application.
