老年患者行前列腺等离子双极汽化术中胶体渗透压以及内环境的变化

摘要 目的观察老年患者行经尿道前列腺等离子双极汽化术的胶体渗透压(COP)以及其他内环境指标的变化。方法120例ASAⅡ~Ⅲ级行经尿道前列腺等离子汽化术的老年患者,于术前(基础值T0)、手术开始后60min(T1)、90min(T2)、120min(13)、150min(T4)取动脉血检测COP、血气分析、血电解质(K+、Na+、HCO3-)、乳酸、血糖以及红细胞压积(Hct),同时记录在各时点的平均动脉压(MBP)、心率(HR)、血氧饱和度(SPO2)、以及中心静脉压(CVP)。结果120例患者中有47例在60min以内完成手术,38例在90min以内完成手术,23例患者在120min左右完成手术,12例患者在150min左右完成手术。T1~T4患者的COP以及Hct逐渐下降并显著低于基础值(T0)(P〈0.05),T2~T4其pH、PaO2、K+、HCO3-、乳酸、血糖和SPO2均随着时间延长逐渐下降并显著低于基础值(P〈0.05),而PaCO2、MBP、HR以及CVP则随着时间延长逐渐上升(P〈0.05),但Na+变化始终不大(P〉0.05)。结论手术时间超过60min以上,COP即开始出现下降,但此时,对老年患者内环境的其他影响较小。若手术时间超过90min以上,则血液出现明显的稀释,有容量超载的可能。 Objective To observe the changes of colloid osmotie pressure(COP) and internal en- vironment in old patients undergoing plasmakinetie vaporization of prostate (PKVP). Methods There were 120 ASA Ⅱ -Ⅲ old patients undergoing PKVP. The arterial blood was taken for COP,blood gas anal- ysis, electrolyte, glucose, hematoerit ( Hct ) and lactic acid at the time points of preoperation ( T0 ), 60 min ( T 1 ) , 90 min ( T2 ) , 120 min ( T3 ) and 150 min ( T4 ) after operation beginning, and mean arterial pressure (MBP), heart rate( HR), SPO2 and central venous pressure(CVP) were recorded at these time points. Re- suits The operation was successfully performed within 60 min in 47 eases, within 60 - 90 min in 38 ca- ses,within 90 - 120 min in 23 eases,and within 120 - 150 min in 12 eases. Compared with T0 ,the level of COP and Het of patients decreased obviously from T1 to T4 ( P 〈 0.05 ) ; The level of PH, PaO2, K + , HCO3 - , glucose laetie acid and SPO2 decreased from T2 to T4 ( P 〈 0.05 ) ; The level of PaCO2, MBP, HR and CVP increased gradually from T2 to T4 ( P 〈 0.05 ) ; The changes of Na + from T2 to T4 were not significant ( P 〉 0.05 ). Conclusion When the time of operation exceeds 60 min, the COP of old patients begins to decrease, but the effect on internal environment is not significant yet. If the time of operation ex- ceeds 90 min, the hemodilution in old patients will become apparent and the volum overload will be possi- ble.
