摘要
黎凯教授主张面瘫急性期(发病≤15d)尽早应用激素抗炎、消水肿、防止神经变性,并通过针刺治疗疏通经气,调和气血用以驱邪外出。恢复期前期(发病>15d~≤30d)在针刺后进行梅花针叩刺,加强体针刺激程度,延长作用时效,加快恢复进程;每穴叩20~30次/次,1次/2d;10次为1疗程,进行1疗程即可。恢复期中期(发病>30d)电针对面瘫恢复中后期,局部恢复不良针对性较强;眼睑闭合困难选阳白-太阳或太阳-下关,口角歪斜选颊车-地仓或颊车-承浆,每次成对选择;逐渐加大刺激量,以出现面部肌肉跳动且患者可忍受为度,脉冲幅度不超过20V,电流不超过2mA,选择疏密波刺激;15min/次,电针停止后继续留针15min。每1次/2d,10次为1个疗程。黎师选择在口服强的松3d后开始针刺,避过神经水肿的高峰期;治疗中除合理使用激素外,神经营养剂也应积极使用,并注意患者情绪的疏导。
