恶性肿瘤化疗致粒细胞减少的研究

Malignant Tumor Chemotherapy Induced Neutropenia Study

目的:探讨恶性肿瘤化疗致粒细胞减少的临床特征。方法:回顾性分析126例化疗后致粒细胞减少患者的临床资料。结果:中性粒细胞正常组院内感染率7.55%,中性粒细胞减少Ⅰ～Ⅲ度组院内感染率20.43%,中性粒细胞减少Ⅳ度组院内感染率44.44%,组间比较有显著性差异(P<0.05或P<0.01);中性粒细胞减少组肿瘤Ⅲ期及胃肠道反应Ⅲ度以上的患者显著多于中性粒细胞正常组,两组比较有显著性差异(P<0.01)。结论:化疗致中性粒细胞减少是造成院内感染率增加最直接的因素,因此,应注意保护好患者的骨髓功能,尽力减少和避免严重骨髓抑制的发生,以期获得更好的化疗效果。

Objective：Malignant tumor patients with chemotherapy induced neutropenia clinical characteristics.Methods：Retrospective analysis of neutropenia induced by chemotherapy of malignant tumor in 126 cases of patients with clinical data.Results：Neutrophils in normal group the nosocomial infection rate in 7.55%,neutropenia I-III degree group rate of nosocomial infection in 20.43%,neutropenia stage group nosocomial infection rate of 44.44%,compared between groups with significant difference（P0.05 or P 0.01）;neutropenia tumor group III and gastrointestinal reaction in patients with III degree more than neutrophils in normal group,two groups had significant difference（P 0.01）.Conclusion：Chemotherapy induced neutropenia is caused by nosocomial infection rates increased most direct factors,therefore,should pay attention to protect the patient’s bone marrow function,try to reduce and avoid the occurrence of severe bone marrow suppression,in order to obtain the better effect of chemotherapy.
