观察硬膜外麻醉阻滞复合瑞芬太尼和异丙酚用于剖胸手术快通道麻醉临床应用效果

Observe Epidural Anesthesia Blocks the Composite Rachel Fentanyl and Propofol Used for Cutting Chest Operation Is Fast Channel Anesthesia Clinical Application Effect

目的:探讨硬膜外麻醉阻滞复合瑞芬太尼和异丙酚在剖胸手术快速麻醉中的应用效果。方法:4例患者Ⅰ～Ⅱ级剖胸手术的患者随机分为观察组和对照组,每组32例,对照组采用普通麻醉,静脉联合硬膜外麻醉,观察组采用快通道麻醉,比较两组患者的围术期血压、心率的变异性,总麻醉时间、麻醉苏醒时间、拔管时间、异丙酚、维库溴铵的用量及术后不良反应。结果:观察组围术期间的心率和血压较对照组的波动小,P<0.05。观察组的麻醉总时间、麻醉苏醒时间及拔管时间显著短于对照组,P<0.05。观察组的异丙酚、维库溴铵的用量显著少于对照组,P<0.05。两组术后24h疼痛、恶心、呕吐、呼吸困难的发生率无显著性差异,P>0.05。观察组术后苏醒期躁动的发生率显著低于对照组,P<0.05。结论:快通道麻醉能够减轻胸部手术患者的应激反应,可确保患者术后迅速苏醒,并减少术后并发症,有利于患者早日康复。

Objective：To study the epidural anesthesia blocks the composite Rachel fentanyl and propofol profile thoracic operation in the application effect of rapid anesthesia.Methods：64 cases were Ⅰ ASA Ⅱ profile thoracic operation level to the patients were randomly divided into trial group and control group,each group of 32 cases and control using common anesthesia,vein joint epidural anesthesia,the observation group using fast channel anesthesia,compared with two sets of patients perioperative blood pressure,and heart rate variability,total anesthesia time,anesthesia time,drawing tube waking time,propofol,vecuronium and the dosage of ammonium and postoperative adverse reactions.Results：The observation group during the pre-operation the heart rate and blood pressure than the control group of the fluctuation of small,P0.05.The observation group of anesthesia total time,anesthesia time and drawing tube revive significantly short time in control group,P0.05.The observation group of propofol,vecuronium significantly less than the amount of ammonium control group,P0.05.Two groups of postoperative 24 h pain,nausea,vomiting,the incidence of breathing difficulties no significant differences,P0.05.The observation group revive the incidence of postoperative period restless significantly lower than the control group,P0.05.Conclusion：Fast channel anesthesia that can reduce chest surgery patients with stress response,ensure the postoperative came quickly,and postoperative complications,beneficial to patients recover soon.
