上海市社区卫生服务机构资源状况及发展研究

The research of resources situation and development of community health service institutions in Shanghai

目的:充分了解上海市社区卫生服务机构的资源及发展状况,为明确社区卫生服务机构今后的发展方向提供依据。方法:对上海市19个区县的社区卫生服务机构2004年至2008年的资源状况进行调研并进行统计分析。结果:主要表现为在收入方面呈增长趋势,其中药品收入占总收入的比例非常高,约58%。各项支出增加,其中,业务支出比重占总支出的60%以上。但收支结余年平均增长率仍为负值。人员方面专科及其以上学历的职工所占比率呈逐年递增趋势,年均增长率达到了10%。但人员培训方面稍显不足,其中,社区护士取得国家级资质证书的年均增长率仅为0.38%,市级培训证书甚至出现了负增长。在设备配置方面,与卫生部的标准还存在一定差距,两者从一定程度上制约了机构的发展。结论:建议进一步出台相关政策,加大对社区卫生服务机构的财政投入力度,加快机构在人员培训以及设备配置方面的建设。

Objective：To have a full understanding of community health service institutions in Shanghai and provide a basis for their future development.Methods： The resources of community health service institutions in 19 districts and counties of Shanghai from 2004 to 2008 were investigated and analyzed.Results： The results mainly manifested in the increasing trend in income,in which the proportion of drug revenues in total revenues was very high,approximately 58%.Each spending was also increased,in which the proportion of operation spending was relatively high,accounting for more than 60% of total expenditures.However,the annual average growth rate of financial balance remained negative.The percentage of personnel with associate degree and above was increasing every year,with an annual growth rate of 10%.However,personnel training was slightly less enough.The average growth of community nurses who received the national qualification certificate was only 0.38%.The number of nurses receiving municipal training certificates even had negative growth.There is also a gap between the current situation and the standards of the device configuration set by the Ministry of Health.Conclusion： It is recommended that relevant policies should be proposed,financial investment for community health service institutions should be increased and the personnel training and equipment configuration should be sped up.
