福辛普利对高血压病患者血浆P物质和神经肽Y水平的影响

摘要 目的 :探讨高血压病患者抗高血压治疗前后血浆 P物质 (SP)和神经肽 Y(NPY)水平的变化和关系。方法 :用放射免疫分析法测定 30例高血压病患者福辛普利治疗前和治疗 3个月后及 2 8例正常对照组的血浆 SP和 NPY水平。结果 :高血压病患者的基础血浆 SP水平明显低于对照组 [(6 4.5 7± 2 2 .5 2 )比 (94.2 8± 2 3.6 4) ng/ L ,P<0 .0 0 1], 期患者低于 期患者 [5 4.0 7± 2 2 .5 6 )比 (73.75± 18.6 1) ng/ L,P<0 .0 5 ,伴左室肥厚 (L VH)者低于无 L VH者 [5 4.33±2 3.98)比 (71.39± 19.2 2 ) ng/ L ,P<0 .0 5 ].而高血压病患者的基础血浆 NPY水平则明显高于对照组 [130 .33± 45 .43)比 (91.96± 2 9.45 ) ng/ L,P<0 .0 0 1], 期患者高于 期患者 [(15 0 .71± 46 .11)比 (112 .5 0± 37.6 8) ng/ L,P<0 .0 1],伴L VH者也高于无 L VH者 [(16 2 .5 0± 41.81)比 (10 8.89± 34 .2 4) ng/ L ,P<0 .0 0 1]。高血压病患者血浆 SP和 NPY水平呈显著负相关 (r=- 0 .80 1,P<0 .0 0 1) ,福辛普利治疗 3个月后 ,血浆 SP水平升至 (73.33± 2 1.87) ng/ L(与治疗前或对照组比 P<0 .0 0 1) ,而血浆 NPY水平降至 (10 8.6 7± 33.39) ng/ L(与治疗前比 P<0 .0 0 1) ,与对照组比 P<0 .0 5 )。结论 :高血压病患者血浆 Objective:[WT5”BZ]To investigate the change and relationship between plasma substance P(SP) and neuropeptide (NPY) levels before and after antihypertensive treatment.[WT5”HZ]Methods:[WT5”BZ]Plasma SP and NPY concentrations were measured by specific radioimmunoassay in 30 patients with essential hypertension before and after three months of treatment with fosinopril and in 28 healthy men and woman as controls.[WT5”HZ]Results:[WT5”BZ]The baseline concentrations of plasma SP in hypertensive patients were lower than those in control group[(64.57±22.52) vs (94.28±23.64)ng/L, P <0.001.The levels in phase Ⅱ paticnts were lower than those in phase Ⅰ patients[(54.07±22.56) vs (73.75±18.61)ng/L, P <0.05.The levels in patients with left ventricular hypertrophy(LVH) were also lower than those in without LVH [(54.33±23.98) vs (71.39±19.22)ng/L, P <0.05]. Howeyer the baseline levels of NPY in hypertensive patients were higher than those in control group[(130.33±45.43) vs .(91.96±29.45)ng/L, P <0.001].The levels in phase Ⅱ patients were higher than those in phase Ⅰ patients [(150.71±46.11) vs .(112.50±37.68)ng/L, P <0.01].The levels in patients with LVH were also higher than those in patients without LVH[(162.50±41.81) vs .(108.89±34.24)ng/L, P <0.01].A negative correlation was found between plasma SP and NPY levels in the hypcrtcnsion group( r =-0.801, P <0.0001).After three months treatment of fosinopril,plasma SP levels increased to(73.33±21.87)ng/L( P <0.001 vs before treatment of controls).whereas plasma NPY levels dereased to(108.67±33.39)ng/L( P <0.001 vs .before treatment, P <0.05 vs .controls.[WT5”HZ]Conclusion:[WT5”BZ]The present study shows that plasma SP levels decreased but plasma NPY levels increased in patients with essential hypertension,particularly in those with LVH.Plasma SP levels are negatively correlated with plasma NPY levels,suggesting that they may be involved in regulating blood pressure and the pathogenesis of hypertensive cardiac hypertrophy.
