老年性白内障囊外摘出并人工晶体植入术后视力低下的分析

Analyses of poor vision after senile cataract extracapsular extraction and intraocular lens implantation

目的 :分析老年性白内障囊外摘出并人工晶体植入术后裸眼视力低下的原因及防治。方法 :对老年性白内障囊外摘出并人工晶体植入术的患者 ,出院时的 6 2 6例 80 5眼 (简称出院组 )和 2个月后复诊的 181例 2 10眼 (简称随诊组 )进行检查 ,对裸眼视力 <0 .5者的原因进行统计分析。结果 :裸眼视力 <0 .5者 ,出院组 2 36眼 (2 9.3% ) ,其中因术前原有眼病所致的 19眼 ,因手术致散光、过矫或欠矫等 2 17眼 ;随诊组 43眼 (5 .3% ) ,后囊混浊 2 2眼 ,散光、过矫或欠矫 14眼 ,角膜失代偿 4眼 ,葡萄膜炎 3眼。结论 :老年性白内障囊外摘出并人工晶体植入术后裸眼视力低下的原因 ,主要是手术带来的屈光不正、后发性白内障等。提高手术技巧或选新术式 ,及时随诊 。

Objective:[WT5”BZ]To analyze the causes and the prevention in low naked vision after senile cataract extracapsular cataract extraction(ECCE)and intraocular lens(IOL)implantation.[WT5”HZ]Methods:[WT5”BZ]805 eyes of 626 cases had been operated with ECCE and IOL in which,210 eyes of 181 cases had been examined after discharge.The causes of those naked vision lower than 0.5 were analyzed statistically.[WT5”HZ]Results:[WT5”BZ]During a shot period of time in the operated group,236 eyes were lower than 0.5 with naked vision(29.3%),among which there were 19 eyes with existing ocular diseases before operation,217 eyes with postoperative astigmatism by over or under correction of the refractive error.In follow up group there were 43 eyes with poor naked vision,among which 14 eyes with postoperative astigmatism by over or under corrective of the refractive error,22 with posterior capsular opacification,4 with corneal endothelial decompensation and 3 with uveitis.Conclusion:After senile cataract ECCE and IOL implantation,the primary causes of low vision in naked eyes were dioptral changes,secondary cataract,corneal edema,etc.Improving operation technique,choosing new methods,and follow up period may play important roles in the prevention of poor vision.
