胰腺损伤性消化道出血临床分析

目的 :探讨胰腺损伤与迟发性消化道出血的病理和临床诊治。方法 :回顾性分析 8例胰腺损伤性消化道出血病例 ,对其病史、临床表现、诊断和治疗及结果进行了总结。结果 :8例中 3例为少量出血 ,5例为大出血 ,其中死亡 3例。上腹外伤病史 6例 ,急性胰腺炎病史 2例。全部病例均伴有胰腺假性囊肿。 结论 :胰腺损伤性消化道出血多伴有胰腺假性囊肿 ,外伤和胰腺炎是其主要原因。诊治困难 ,死亡率高 ,应尽早积极处理。对不明原因的消化道出血病人 ,CT检查发现胰腺假性囊肿时应高度怀疑假性囊肿出血。

Objective: To investigate the features of pancreas damage and haemorrhage of digestive tract. Method: Eight patients with tardy haemorrhage of digestive tract were studied retrospectively. Results: Three of eigth patients were of small quantity of haemorrhage and the others were of massive haemorrhage in which three patients were died. Six of eight patients had the history of trauma in epigastrium and two patients had the history of acute pancreatitis. Conclution: The tardy haemorrhage caused by pancreas damage are in company with pancreas pseudocyst in which the primary cause are pancreas damage and pancreatitis; fatality is high in this disease. CT examination is helpful for diagnosis.
