Ⅲ期乳腺癌术前尺动脉插管灌注化疗的远期疗效

Long-term effect on preoperative ulnaris arterial intubation chemotherapy in treatment of stage Ⅲ breast cancer

目的 探讨术前尺动脉插管灌注化疗在治疗Ⅲ期乳腺癌中的临床价值。 方法 同期治疗Ⅲ期乳腺癌患者 10 9例 ,其中术前行尺动脉插管灌注化疗 6 4例 ,称A组 ;术前未化疗 45例 ,称B组。观察尺动脉插管灌注化疗对A组患者的疗效 ,并对 2组患者 5、10年生存率及无病生存率进行对比分析。 结果 A组患者的总有效率 76 6 % ,组织学有效率为 89 1% ;5、10年生存率分别为6 4 0 %、47 9% ;5、10年无病存活率分别为 5 9 4%、43 5 % ,均明显高于B组的 5、10年生存率 (31 1%、2 1 9% )及 5、10年无病存活率 (2 2 2 %、12 5 % ) ,差异有显著性意义 (P <0 0 5 )。 结论 术前尺动脉插管灌注化疗是治疗Ⅲ期乳腺癌的一种有效辅助措施 ,能够提高Ⅲ期乳腺癌患者的远期疗效 ,并减少局部复发和远处血行转移。

Objective[WT5”BZ] To evaluate preoperative ulnaris arterial intubation chemotherapy as a step of multidisciplinary treatment of stage Ⅲ breast cancer. [WT5”HZ]Methods[WT5”BZ] 109 patients with stage Ⅲ breast cancer were studied. 64 patients (group A) were treated by preoperative ulnaris arterial intubation chemotherapy and others (group B) were not. The 5 and 10 year survival rates and disease free survival rates were analyzed. [WT5”HZ]Results[WT5”BZ] In patients with stage Ⅲ breast cancer, the overall response rate of group A was 76 6%, and the response rate to histological study was 89 1%. The 5 year and 10 year survival rates of group A were 64 0% and 47 9% respectively. The disease free survival rate of group A was 59 4% at 5 years and 43 3% at 10 years. All were higher than the 5 year and 10 year survival rates(31 1%、21 9%) and the 5 year and 10 year disease free survival rates (22 2%、12 5%) of group B ( P <0 05). [WT5”HZ]Conclusion[WT5”BZ] Preoperative ulnaris arterial intubation chemotherapy is an effective measure for the treatment of stage Ⅲ breast cancer. It can improve the long term effects on stage Ⅲ breast cancer and reduce local relapse and distant metastasis. [WT5”HZ]
