Xanthogranulomatous pyelonephritis

目的 探讨黄色肉芽肿性肾盂肾炎 (XGP)的诊断和治疗。 方法 分析总结 2 8例XGP患者临床特征、诊断和治疗的资料。 结果 XGP的临床特征 :(1)尿路有结石或梗阻因素存在 ,患肾功能受损害 ;(2 )常合并尿路感染 ;(3)贫血、血沉增快、肝功能异常。 13例术前诊断。 2 0例行患肾切除 ,8例作保肾手术。术后随诊患者恢复良好。 结论 XGP的临床特征和影像学检查有助于术前诊断 ,并可与肾肿瘤和其它肾脏感染性疾病鉴别。外科手术和抗感染是XGP治疗的重要方法。Ⅲ期患者应行患肾切除 ,Ⅰ、Ⅱ期患者可采用保肾手术。

Objective[WT5”BZ] To study the diagnosis and treatment of xanthogranulomatous pyelonephritis(XGP). [WT5”HZ]Method[WT5”BZ] The clinical characteristics, diagnosis and therapy of 28 cases of XGP were discussed. [WT5”HZ]Results[WT5”BZ] The clinical characteristics of XGP included: calculi or obstruction in urinary tract, and damage to kidney; complication of urinary tract infection; anemia,increased ESR and dysfunction of liver. 13 patients with XGP were diagnosed before operation. Nephrectomy was performed in 20 patients. Kidneys were perserved in 8 patients. All patients recovered after operation. [WT5”HZ]Conclutions[WT5”BZ] The clinical characteristics and image of XGP are good to preoperative diagnosis. They are useful to differentiate XGP from renal carcinoma and other renal infectius disease. Surgery and antimicrobial are important to XGP. Nephrectomy might be done in patients of stage Ⅲ. kidney may be preserved in patients of stageⅠ and stageⅡ. [WT5”HZ]
