404名中医医师高血压病中医临床实践指南使用情况的调查研究

Implementation of clinical practice guideline on traditional Chinese medicine for hypertension:a survey on 404 clinical practitioners

目的:本研究基于前期对104名中医医师使用高血压病中医临床实践指南调查研究,扩大了中医医师调查范围,旨在深入了解中医临床实践指南的使用情况和存在问题,并提出中医临床实践指南的推广建议。方法:采用问卷调查的方法,对7个地区404名高血压病中医专科医师及基层全科医师(中医背景)在临床工作中使用高血压病中医临床实践指南的情况进行调查研究,使用频数统计、卡方检验及Logistic回归分析对数据进行处理。结果:在被调查的医师中68.81%的中医医师曾经在临床诊疗过程中使用过高血压病中医诊疗指南,88.89%的中医医师对中医指南持认可态度,并愿意在今后的工作中使用中医指南,"技术职称"、"学历"、"医院级别"、"从业时间"不同的中医医师使用指南的情况和对指南的态度存在统计学差异,目前影响中医指南使用的主要原因认识推广宣传不足。结论:本研究与前期研究成果一致,大部分中医医师在临床工作中使用过中医临床实践指南并对中医临床实践指南持认同态度,但中医指南的推广应用仍十分缺乏,应建立中医临床实践指南多种发布渠道,针对特定中医医师人群开展专门培训,并结合优越政策环境,加大中医指南推广宣传力度。

Objective： To assess implementation of clinical practice guideline on traditional Chinese medicine（TCM） for hypertension.Methods： We used questionnaire survey to evaluate 404 general practitioners and hospital-based specialists from 7 areas with the object of exploring implementation situation toward clinical practice guidelines（CPG） on TCM for hypertension,and processed datas with frequency statistics,chi-square test and logistic regression analysis methods.Results： 68.81% TCM doctors used CPG in their clinical works.The 88.89% TCM doctors had positive attitude on TCM CPG and would like to use TCM CPG in the future.The usage rate of TCM CPG was different due to various grades of hospital,technical title,clinical times and education background of TCM doctors.Shortage of popularization of guideline was the most important factors which influence the implementation of clinical practice guideline on TCM.Conclusion： To popularize TCM clinical practice guideline,we suggest that： ①Different ways to release TCM CPG should be established.②More training course for young TCM doctors of how to use TCM CPG should be developed.③Useing the policy to push the implement of TCM CPG.
