原发性干燥综合征中医证型特点的研究

Research of the Chinese Medicine Pattern/Syndrome Characters of Primary Sjogren Syndrome

目的探讨原发性干燥综合征(PSS)中医证型分类特点,分析各证型的分布及其与病情轻重程度的关系。方法以横断面调查方式,制定统一调查表,收集200例PSS患者基本信息、中医症状体征及主要实验室指标,统计分析各证型的分布及各证型与病程、系统损害情况、中医症状体征评分、实验室指标的关系。评价各证型病情轻重程度。结果 200例中医证型中单证占32%;复合证型占53%。其中阴虚血瘀证34例(17%)最多、气虚证30例(15%)次之、气阴两虚证26例(13%)再次之。不同年龄和病程组证型复杂程度差异无统计学意义(P>0.05)。中医症状体征评分和年龄、病程呈正相关,r值分别为0.213、0.168。中医症状体征评分和血沉、免疫球蛋白水平无相关性。结论 PSS以复合证型为主,阴虚血瘀证最多。PSS中医症状体征评分和年龄及病程呈正相关,气虚血瘀证最重。

Objective To explore the characters of Chinese medicine pattern/syndrome differentiation of primary Sjogren syndrome（PSS）and analyze the distribution of the patterns/syndromes as well as the correlation with the disease severity.Methods The cross-sectional survey method was used to implement the standard investigation form.The general information,Chinese medicine symptoms and physical signs and the main laboratory indicators of 200 cases of PSS were collected.The distribution of each pattern/syndrome and the correlation with the duration of illness,the situation of systematic damage,the score of Chinese medicine symptoms and physical signs and the laboratory indicators were analyzed statistically.The disease severity of each pattern/syndrome was assessed.Results Of 200 cases,the single pattern/syndrome of Chinese medicine accounted for 32% and the complex pattern/syndrome for 53%,of which,there were 34 cases（17%）of yin deficiency and blood stagnation pattern/syndrome,30 cases（15%）of qi deficiency pattern/syndrome and 26 cases（13%）of qi and yin deficiency pattern/syndrome,occupying the first three tops in the list successively.The difference in the complex degree was not significant statistically among the groups with different age and duration of illness considered（P0.05）.The correlation of the score of Chinese medicine symptoms and physical signs with age and duration of illness considered was positive,r=0.213,0.168 respectively.The score of Chinese medicine symptoms and physical signs was not correlative with the levels of erythrocyte sedimentation and immune globulin.Conclusion PSS is differentiated as the complex patterns/syndromes of Chinese medicine,commonly as yin deficiency and blood stagnation pattern/syndrome.The score of Chinese medicine symptoms and physical signs of PSS is correlative positively with age and duration of illness,in which qi deficiency and blood stagnation pattern/syndrome is considerable.
