盘内注射胶原酶治疗腰椎间盘突出症介入术后疼痛的临床观察

Clinical Observation of Postoperative Pain in Treatment of Lumbar Disc Herniation by Intervertebral Injection of Collagenase

目的分析盘内注射胶原酶治疗腰椎间盘突出症介入术后疼痛的临床疗效。方法将54例腰椎间盘突出症患者随机分成观察组和对照组,各27例,两组患者均接受盘内注射胶原酶治疗,观察组于术后第2天行硬膜外封闭,对照组除未进行硬膜外封闭外,其他治疗措施与观察组相同。观察记录两组患者的术后疼痛情况。结果观察组术后疼痛持续时间短于对照组(P<0.05)。54例患者均经定期随访或随诊,时间6～12个月,平均7.5个月。两组病例早期效果进行比较差异有统学意义(P<0.05)。结论临床上应掌握好适应证,将胶原酶和硬膜外封闭结合使用,可使其缺点消除,优点互补,取得较佳疗效。

Objective To analyze the clinical efficacy of intervertebral injection of collagenase on the postoperative pain in the treatment of lumbar disc herniation.Methods A total of 54 patients with lumbar disc herniation were randomly divided into observation group and control group,with 27 cases in each group.All patients received intervertebral injection of collagenase.Patients in the observation group underwent epidural block at 2 days post-surgery.The other therapeutic measures were the same between the observation group and control group.Postoperative pain in patients of two groups was observed.Results The degree of postoperative pain in observation group was lower than that in control group（P0.05）.All 54 patients received regular follow-up or visits for 6-12 months,7.5 months on average.There were significant differences in the early curative effects between the two groups（P0.05）.Conclusion Clinical indications should be closely observed,the combination of collagenase and epidural blocking can overcome its shortcomings and have complementary advantages,thus achieving a better efficacy.
