盐酸吡格列酮治疗糖尿病脑病的临床疗效观察 被引量:3

摘要 目的评价盐酸吡格列酮治疗糖尿病脑病患者认知功能的疗效,并探讨其影响机制。方法将40例糖尿病脑病患者随机均分为盐酸二甲双胍治疗组(C组)和盐酸二甲双胍+盐酸吡格列酮治疗组(D组),治疗前后分别采用简易精神状态量表(MMSE)观察患者认知功能改变,并与正常对照组(A组)及有糖尿病无脑病组(B组)进行比较。对各组治疗前后清晨抽取受试者空腹静脉血测定血清胰岛素样生长因子(IGF-1)、β淀粉样蛋白(Aβ1-42)水平。结果 A、B组MMSE评分正常,C、D组治疗前均有明显认知功能障碍,C组治疗前后MMSE评分差异无统计学意义,D组治疗后(18.2±0.4)较治疗前(15.3±0.5)评分增加、认知功能障碍改善(P<0.05)。C、D组治疗前IGF-1、Aβ1-42水平无明显差异,A、B、C组治疗前后IGF-1、Aβ1-42水平无明显差异,D组治疗后IGF-1水平较前升高,治疗后D组(105.54±23.81)高于C组(94.87±39.02),比较差异有统计学意义(P<0.05)。D组治疗后Aβ1-42水平较前降低,治疗后D组(0.535±0.007)低于C组(0.714±0.123),比较差异有统计学意义(P<0.05)。结论盐酸吡格列酮可能通过升高IGF-1、降低Aβ1-42的表达改善糖尿病脑病患者认知功能。
