摘要 失效模式与效应分析（failure mode and effect analysis，FMEA）最早使用于20世纪50年代美国宇航和军方，作为一种系统性和前瞻性的质量管理工具，用于持续改进产品质量。90年代中期以后，美国把这一管理方法开始应用于预防配液过程中的差错，并于2002年起，美国医疗机构联合评审委员会正式将FMEA应用于医疗机构，推广FMEA方法，
