股骨近端锁定板与动力髋螺钉治疗老年股骨粗隆间骨折效果比较 被引量:1

Effects comparison of femoral proximal locking plate and dynamic hip screws fixation in the treatment of elderly patients with intertrochanteric fracture

摘要 目的探讨临床应用股骨近端锁定板(LPFP)和动力髋螺钉(DHS)治疗老年股骨粗隆间骨折的手术疗效。方法回顾性分析老年股骨粗隆间骨折46例患者的临床资料,LPFP组和DHS组各23例。分析比较两种术式平均手术时间、术中出血量、下地时间、完全负重时间、住院时间、骨折愈合时间及术后功能恢复方面的差异。结果LPFP组与DHS组的手术时间、术中出血量、完全负重时间、骨折愈合时间比较差异有统计学意义(P〈0.05),LPFP组优于DHS组。平均住院时间分别为20d(LPFP组)和28d(DHS组)。对于稳定性股骨粗隆间骨折,两组优良率比较差异无统计学意义(P〉0.05);对于不稳定性股骨粗隆间骨折,LPFP组的优良率优于DHS组。结论对于老年股骨粗隆间骨折的治疗,LPFP优于DHS。 Objective To investigate the efficacy of the proximal femur locking plate (LPFP) and the dynamic hip screw(DHS) treatment of elderly patients with intertrochanterie fracture. Methods The clinical data of 46 cases of elderly patients with intertrochanteric fracture were retrospectively ana- lyzed, and they were divided into LPFP group and DHS group, 23 cases in each group. Comparing two groups mean operation time, intraoperative blood loss, ambulation time, full weight bearing, length of stay, fracture healing time and postoperative functional recovery differences. Results Operation time, bleeding volume, full weight bearing and fracture healing time between the two groups, the difference was statistically significant (P 〈 0. 05 ), LPFP group was superior to the DHS group. The mean duration of hospitalization were 20 days (LPFP) and 28 days(DHS). The stability of femoral intertrochanteric frac- ture in the two groups, the excellent and good rate had no significant difference, for instability intertro- chanteric fracture LPFP group was superior to the DHS group. Conclusions For elderly patients with in- tertrochanteric fracture, treated with LPFP is superior to DHS.
