肾损伤分子-1测定对诊断过敏性紫癜早期肾损害的意义

Implications for Measurement of Urinary Kidney Injury Molecule-1 in Diagnosis of Anaphylactoid Purpura Renal Injuries

摘要 目的探讨肾损伤分子(Kim-1)测定对诊断过敏性紫癜早期肾损害的意义。方法研究对象分为3组:过敏性紫癜(HSP)无尿改变组30例、HSP有尿改变组30例、对照组30例。采用ELISA方法检测尿Kim-1、mAlb,比色法检测尿NAG酶,放射免疫法检测尿β2-MG水平。结果 HSP有尿改变组尿Kim-1、mAlb、NAG酶、β2-MG水平分别高于HSP无尿改变组及健康对照组,差异有统计学意义(P<0.05);HSP无尿改变组高于健康对照组,差异有统计学意义(P<0.05)。HSP有尿改变组中,尿Kim-1与其他3项阳性率比较,差异有统计学意义(P<0.05)。结论Kim-1可作为反映早期HSP肾损伤的敏感指标。 Objective To investigate the clinical significance of detecting urinary kidney injury molecule-l(Kim-1) in early diagnosis of anaphylactoid purpura renal injuries. Methods Sixty children with Henoch-Schonlein purpura (HSP) were divided into two groups.no urinary-change group and urinary-change group, with 30 children in each group. In addition, 30 healthy subjects were selected as normal controls. The urinary Kim-1 and microamount albumin (mAlb) levels were detected by ELISA, urinary N-acetyl-beta-D-glucosaminidase (NAG) levels by colorimetric method, and 132 microglobulin (β2-MG) by radioimmunoassay. Results The levels of Kim-1, mAIb,NAG and β2-MG in urinary-change group were significantly higher than those in no urina- ry-change group or control group (P 〈 0.05). Moreover, these parameters in no urinary-change group were obviously higher than those in control group(P〈0.05). In urinary-change group,the positive expression rates of urinary Kim-1 were remarkably higher than those in other three groups(P〈0.05). Conclusion Kim-1 is a sensitive marker for early renal damage in children with HSP.
