摘要
目的对42例血尿型IgA肾病患者部分尿肾小管损伤相关指标进行检测分析,以了解血尿型IgA肾病患者肾小管的损伤情况。方法以42例肾脏活组织病理检查确诊的血尿型IgA肾病患者为观察组,同时选取42例健康体检者为对照组。两组常规留尿检测乙酰β-D氨基葡萄糖苷酶(NAG)、视黄醇结合蛋白(RBP)、β2-微球蛋白(β2-M)、尿蛋白肌酐比值(ACR)、红细胞计数(RBC)及渗量的变化。结果观察组尿NAG、β2-M、RBP均较对照组为高,尿渗量两组间无统计学意义;且尿RBC与NAG、β2-M、RBP、渗量均无相关性。结论血尿型IgA肾病患者存在着明显的近端肾小管损伤,且损伤程度与尿红细胞数多少无关。
Objective To investigate changes of urinary biomarkers of tubular injury in 42 single hematuria patients with IgA nephropathy in order to study renal tubular damage degree. Methods Forty two patients with biopsy-proved idiopathic single hematuria patients with IgA nephropathy were recruited in this study. 42 healthy persons were regarded as the control group. The changes of urinary N-acetyl-β-D glucosaminidase (NAG), retinol binding protein (RBP), Beta2-microglobulin (β2-M), albumin creatinine ratio (ACR), red blood cell count (RBC) and urine osmotic pressure (UOSM) were detected in two groups. Results Urinary NAG, RBP, β2-M, RBC were increased in single hematuria patients with IgA nephropathy compared with healthy persons, except UOSM and ACR. Urinary RBC counts were no correlated significantly with urinary NAG, RBP, β2-M and UOSM. Conclusion Single hematuria patients with IgA nephropathy has obviously proximal tubular damage, which injury degree has nothing to do with urinary RBC counts.
