自由体位对足月胎膜早破孕妇分娩结局的影响

Effect of liberal position on the maternal and neonatal outcomes of full-term pregnant women with premature rupture of membrane.

目的探讨自由体位对足月胎膜早破孕妇分娩结局的影响。方法 2009年1月至2012年1月在我院产科进行住院待产的120例足月胎膜早破孕妇,按照入院的顺序随机分为实验组和对照组各60例,在监测胎心、胎动、定时吸氧情况下,实验组采取不限制孕妇体位,让孕妇取自由舒适体位待产;对照组行传统的抬高臀部卧床休息待产。结果实验组第二产程及总产程均明显低于对照组,差异有统计学意义(P<0.01);新生儿窒息、尿潴留等发生率实验组明显低于对照组,差异均有统计学意义(P<0.05)。结论足月胎膜早破的产妇采取自由体位待产更有利于促进产程进展,减少并发症的发生。

Objective To investigate the effect of liberal position on the maternal and neonatal outcomes of full-term pregnant women with premature rupture of membrane. Methods One hundred and twelve full-term preg- nant women with premature rupture of membrane in the Department of Obstetric and Gynecology from January 2009 to January 2012 were randomly divided into the study group and the control group, with 60 cases in each group. With the monitoring of fetal heart and movement as well as oxygen inhalation, the study group adopted liberal position and let the pregnant woman take free comfortable posture for labor, while the control group applied the traditional hip ele- vation. Results The second stage of labor and the birth process in the study group were both significantly lower than those in the control group （P〈0.01）. The incidence of neonatal asphyxia and that of urinary retention in the study group was significantly lower than those in the control group （P〈0.05）. Conclusion Liberal position can help full-term pregnant women with premature rupture of the membrane promote the progress of labor and reduce the inci- dence of complications.
