颅骨脑膜瘤临床特点及诊治分析

Analysis of the clinical characteristics and the diagnosis and treatment methods for lmeningioma of the skull

目的提高对颅骨脑膜瘤侵袭性生长特点的认识,形成一旦确诊,需彻底超范围切除的手术理念,减少复发。方法回顾分析我院收治的2例颅骨脑膜瘤,就其生长特点、诊断、病理特点及手术切除范围结合相关文献进行分析。结果本组2例患者1例术前诊断为颅骨骨瘤,1例术前诊断为颅骨脑膜瘤,术中对受累颅骨、硬膜均行超范围切除,术后病理均证实为颅骨脑膜瘤。结论颅骨脑膜瘤为良性肿瘤,手术切除没有技术难点,然而该型肿瘤却具有局部易复发,对骨质、硬膜甚至脑组织侵蚀破坏以及耐受放疗等恶性生长特点,所以彻底超范围切除极其重要。

Objective To enhance the cognition of the characteristics of the progressive growth of the skull meningioma,in order to form the surgery concept that once diagnosed, an immediate and radical and beyond resection is necessary so as to reduce the recurrence. Methods Two cases of meningioma of the skull from January 2010 to June 2012 were analyzed retrospectively in respect of growing characteristics, diagnosis, pathological characteristics and resection range combined with related literature. Results One of two cases is diagnosed as osteoma of skull and the other is diagnosed as meningioma of the skull preoperatively. Resection of the involved bone and dura was beyond and two cases were confirmed meningioma of skull by surgical pathology completely. Conclusion these tumors are classified as benign and surgical resection is no technical difficulties,however,characteristics of malignant growth of local recurrence easily, invasion of bone,dura and brain tissue, and radioresistance make treatment treacherous and it is vitally important that these tumors must be beyond to resect completely.
