经直肠彩色多普勒超声诊断直肠肿瘤的临床价值

The Clinical Value of Transrectal Color Doppler Ultrasound Used for Diagnosing Rectal Tumors

目的探讨经直肠彩色多普勒超声诊断直肠肿瘤的临床价值。方法对115例直肠肿瘤患者施行超声检查,根据直肠肿瘤的二维声像图、彩色多普勒血流参数特征诊断,并与术后病理学结果进行对照分析。结果 115例直肠肿瘤中,恶性肿瘤87例,与病理符合率89.7%(78/87例);良性肿瘤28例,与病理符合率64.0%(18/28例),总的诊断符合率为83.5%(96/115例)。根据直肠肿瘤二维声像图特征、彩色多普勒血流信号、血流动力参数即动脉的收缩期峰值流速(PSV)及舒张末期流速(EDV)、阻力指数(RI),与手术病理学结果对照,血流信号良性组与恶性组间差异有统计学意义(P<0.05),良性组与恶性组的PSV、EDV间差异无显著性意义(P>0.05)。良性组与恶性组的RI间有统计学意义(P<0.05)。结论经直肠彩色多普勒超声对直肠良恶性肿瘤的鉴别诊断有一定的临床价值。

Objective To study the clinical value of Transrectal Color Doppler Ultrasound used for diagnosing rectal tumors. Methods Base on the comparative analysis the two dimensional Ultrasonography of rectal tumors and the feature of Color Doppler hemodynamic parameter of rectal tumors with the postoperative pathological result. Results In 115 cases of rectal tumors, 87 cases are malignant tumors and 28 eases are benign tumors. In the 87 cases of malignant tumors, the contrast is made between the Ultrasound diagnosed cases and postoperative pathological results, and the coincident rate is 89.7% （78/87）. In the 28 cases of benign tumors, the contrast is made between the Ultrasound diagnosed cases and postoperative pathological results, and the coincident rate is 64. 0% （18/28）. The general diagnostic coincident rate is 83.5% （96/115）. Comparative analyze the feature of the two dimensional of tumors, the characteristic of Doppler Blood Flow Signal and bemodynamic parameter （PSV, EDV, RI） with the postoperative pathological. The difference of Blood Flow Signals between the benign and malignant groups has its prominent Clinical Significance （P〈 0.05）. However the contrast of the hemodynamic parameter （PSV, EDV, RI） is made between the benign and malignant groups, the difference in PSV and EDV does not have its prominent Clinical Significance （P〉0. 05）, but the difference in RI has its prominent Clinical Significance （P〈0.05）. Conclusions Transrectal Color Doppler Ultrasound in the diagnosis of benign and malignant neoplasms have a certain value provides a direct, simple and safe manner.
