非酒精性脂肪性肝病并发结直肠粘膜癌变的临床特征

Clinical characteristics of colorectal injuries in patients with non-alcoholic steatohepatitis

目的探讨NAFLD患者并发结直肠粘膜病变的临床特征。方法在201例NAFLD患者和861例接受肠镜检查者,观察结直肠粘膜病变的病理类型与临床特征。结果 NAFLD患者结直肠病变的检出率为42.8%,显著高于对照组32.9%(P<0.01);其中管状腺瘤及伴中度以上不典型增生者均较对照组升高(P<0.01或P<0.05),女性发病年龄高峰较男性提前。结论 NAFLD患者易并发结直肠粘膜病变,应定期进行全结肠镜筛查。

Objective To examine the prevalence and clinical characteristics oi colorectal injuries in patients with NAFLD. Methods 861 subjects and 201 patients with NAFLD were recruited for enteroscopic screening. The NAFLD was proven by ultrasonic examination. Result The prevalence of colorectal adenomas （42.8% ,P〈0.01）, tubular adenomas （P〈0.01）and obvious dysplasia （P〈0.05） in patients with NAFLD were much higher than those in control subjects;The peak age of onset in women was earlier than in men. Conclusion ＇ NAFLD is associated with a high prevalence of coloreetal injuries and the coloreetal screening by enteroscopy is strongly recommended in patients with NAFLD.
