兰州市3042例乳腺癌患者直接经济负担趋势分析

Trend analysis on direct economic burden of 3 042 patients with breast cancer in Lanzhou city

目的:分析兰州市3 042例乳腺癌患者直接经济负担的变化趋势,为有效控制医疗费用提供科学依据。方法:收集1996～2006年兰州市两家省级医院所有确诊的乳腺癌患者的病历资料,获取住院费用信息并分析直接经济负担趋势。结果:1996～2006年连续11年不同年龄组乳腺癌患者发病数量总体呈上升趋势,平均涨幅为9.21%。连续11年乳腺癌患者平均人均住院费用12 298.97元,平均住院33.97天,总体呈下降趋势,降幅分别为3.09%和2.80%;平均日均住院费用482.95元,涨幅为4.14%,其中2006年的平均住院日和日均费用为1996年的0.63倍和2.12倍;65岁以上乳腺癌患者的人均费用呈上升趋势。结论:35～45岁和>65岁组患者日均住院费用上升显著,提示兰州市乳腺癌患者的经济负担仍然较重。应逐步加强对高危人群乳腺癌的筛查。

Objective： To analyze the changing trend of direct economic burden of 3 042 patients with breast cancer in Lanzhou cit- y, and provide a scientific basis for controlling medical expenses effectively. Methods： The clinical data of all the patients diagnosed as breast cancer definitely in two hospitals at provincial level in Lanzhou city from 1996 to 2006 were collected, and the information about hospi- talization costs was obtained, then the trend of direct economic burden was analyzed. Results： The total number of patients with breast canc- er from 1996 to 2006 showed an increasing trend, the mean increasing amplitude was 9. 21%. The per - capita hospitalization cost of patients with breast cancer from 1996 to 2006 was 12 298.97 Yuan, the average hospitalization time was 33.97 days, showing a decreasing trend as a whole, the decreasing amplitudes were 3.09% and 2. 80% , respectively; the average hospitalization cost daily was 482.95 Yuan, the in- creasing amplitude was 4. 14%, The mean hospitalization time and cost daily in 2006 were 0. 63 times and 2. 12 times more than those in 1996 ; the per - capita hospitalization cost of patients with breast cancer more than 65 years old showed an increasing trend. Conclusion： The average hospitalization costs daily in patients aged 35 - 45 years old and more than 65 years old increase significantly, which indicate that the economic burden of patients with breast cancer in Lanzhou city is still heavy; screening among high risk population of breast cancer should be strengthened gradually.
