乙酰肝素酶、碱性成纤维细胞生长因子的表达与子宫内膜癌浸润、转移及预后的关系

Relationship between expressions of heparanase and basic fibroblast growth factor and invasion,metastasis and prognosis of endometrial carcinoma

目的:探讨乙酰肝素酶、碱性成纤维细胞生长因子的表达与子宫内膜癌浸润、转移及预后的关系。方法:选择2008年3月～2010年10月期间,在该院切除并存档的子宫内膜癌患者子宫内膜石蜡包埋组织标本48例作为研究组,另选同期该院妇产科收治的其他疾病患者,手术切除或刮宫的子宫内膜标本34例作为比较组,及正常子宫内膜21例作为对照组,分别比较分析Hpa和bFGF的染色强度及表达阳性率。结果:与FIGOⅠ期相比,Ⅱ期和Ⅲ期患者Hpa和bFGF的阳性率明显提高;与G1分化相比,G2分化患者Hpa和bFGF的阳性率明显提高;与无肌层浸润相比,肌层浸润浅部和深部患者Hpa和bFGF的阳性率明显提高,差异均具有统计学意义(P<0.05);与对照组患者相比,比较组患者和研究组患者的Hpa和bFGF阳性率均明显提高,差异均具有统计学意义(P<0.05),且研究组的阳性率高于比较组。结论:乙酰肝素酶和碱性成纤维细胞生长因子的表达与子宫内膜癌浸润、转移、预后密切相关,并且有望成为子宫内膜癌联合诊断和病情评估以及疗效观察和预后预测的重要检测指标,同时也可能为子宫内膜癌的治疗提供一种更为有效的解决方案。

Objective：To explore the relationship between expressions of heparanase （Hpa） and basic fibroblast growth factor （bF- GF） and invasion, metastasis and prognosis of endometrial cancer. Methods：Four - eight paraffin - embeded endometrial samples from the patients with endometrial cancer who received surgery and were recorded in the hospital from March 2008 to October 2010 were selected as study group, while 34 paraffin - embeded endometrial samples from the patients with other endometrial diseases who received surgery and u- terine curettage in the hospital during the same period were selected as comparison group, 21 normal paraffin - embeded endometrial samples were selected as control group ; the staining intensities and positive expression rates of Hpa and bFGF in the three groups were compared and analyzed. Results： Compared with FIGO stage [ , the positive rates of Hpa and bFGF at FIGO stage II and FIGO stage Ill increased signifi- cantly; compared with G1 differentiation, the positive rates of Hpa and bFGF of G2 differentiation increased significantly; compared with ca- ses without myometrial invasion, the positive rates of Hpa and bFGF in cases with superficial and deep myometrial invasion increased signifi- cantly, there were statistically significant differences （ P 〈 0. 05 ）. Compared with control group, the positive rates of Hpa and bFGF in com- parison group and study group increased significantly, there was statistically significant difference （P 〈 0. 05 ） , the positive rates of Hpa and bFGF in study group were higher than those in comparison group. Conclusion：The expressions of Hpa and bFGF are closely correlated with invasion, metastasis, and prognosis of endometrial cancer, which may become important indexes to diagnose endometrial cancer, evaluate condition of patients, observe curative effect, and predict prognosis, meanwhile, Hpa and bFGF may provide a more effective scheme for cu- ring endometrial cancer.
