产后口服避孕药对内分泌的影响

The effect of oral contraceptive after parturition on endocrine system

为评估口服避孕药对产妇内分泌的影响,我们对一些相关文献进行了综述。多数研究认为单孕激素类口服避孕药(oral contraceptive,OC)对泌乳没有影响,而复合型避孕药(combination-type oral contraceptive,COC)可能会减少泌乳量或者缩短泌乳时间;大部分研究提示OC对婴儿的生长发育没有影响,而有些数据表明OC影响婴儿发育;对于何时开始使用OC的时间存在争议。故我们认为目前有限的临床资料不足以证明OC对泌乳持续时间和婴儿健康有无影响,世界卫生组织(World Health Organization,WHO)和美国妇产科医师学会(American College of Obstetricians and Gynecologists,ACOG)推荐最早产后6周才能开始使用雌孕激素复合型避孕药。

We have reviewed recent literatures in order to estimate the effect of oral contraceptive （OC） on lactation in postpartum women. Most of the researches demonstrated that progestin OC do not affect lactation, but combination - type oral contraceptive （COC） may reduce milk volume or shorten the lactation time. Little detrimental effects have been shown on infant growth or development with the use of progestin OC, and there was much controversy over the starting time. So we think the evidence is not adequate to determine whether a mother＇s use of these drugs affects lactation duration or the infant＇s health. World Health Organization （WHO） and American College of Obstetricians and Gynecologists （ACOG） recommend parturients take COC at least 6 weeks later.
