中药分期论治验方治疗儿童哮喘45例临床观察

Clinical Observation on Stage Treatment with Experienced Chinese Herbal Formulas for 45 Cases of Pediatric Asthma

目的观察中药分期论治验方治疗儿童哮喘的临床疗效。方法 75例哮喘患儿随机分为中药组45例和西药组30例,另选正常组10例。中药组急性发作期予射干2号合剂,每次10ml,每日3次,7天为1个疗程;缓解期予黄芪补肾合剂为主随症加减治疗3个月。西药组急性发作期予爱纳灵1/2片或1片,每日2次,7天为1个疗程,缓解期予顺尔宁4～5mg,每日1次,治疗3个月。治疗前后对两组患儿进行中医证候评分和西医症状评分,并对中药组和西药组部分患儿和正常组进行肺功能检测。结果两组患儿治疗后中医主要症状积分和总积分、西医主要症状积分和总积分均较治疗前明显降低(P<0.01);治疗后西药组患儿鼻部症状评分低于中药组(P<0.05)。中药组在治疗后通气功能各指标与治疗前比较均有明显改善(P<0.05),且基本达到正常组水平(P>0.05),其中用力肺活量(FVC)的提高优于西药组(P<0.05)。中药组治疗后的肺潮气呼吸、气道阻力中的多项指标得到改善(P<0.05)。结论中药分期论治验方能有效控制哮喘患儿的哮喘发作症状、改善肺功能。

Objective To observe the therapeutic effect of stage treatment with experienced thmese herbat tormmas on peru- attic asthma. Methods Seventy five children with asthma were randomized into the Chinese medication group （45 cases） and western medication group （30 cases）. Another 10 healthy children were chosen as the normal control group. No. 2 Shegan （BelamcandaeRhizorna） Mixture was given to the children in the Chinese medication group at the acute stage, 10ml once, three times daily and for 7 days as one treatment course. Children in the Chinese medication group at the remission stage were treated with Huangqi Bushen Mixture （Astragalus Mixture for Kidney-Supplementing） for 3 months. The children in the west- ern medication group at the acute stage were given 1/2 or 1 tablet of etinoline, twice daily and for 7 days as one treatment course. The children in the western medication group at the remission stage were treated with 4rag to 5mg of singulair, once daily, {or 3 months. The Chinese medicine syndromes and western medicine symptoms were scored and lung function was de- tected be{ore and after treatment in both groups. Results The total scores of main syndromes and symptoms and the total scores were significantly decreased in both groups after treatment （P^0.01）. The scores of nasal symptoms in the western medication group were significantly lower than those in the Chinese medication group after treatment （P^0.05）. The indica- tors of ventilatory function were significantly improved in the Chinese medication group after treatment （P〈0.05）, reaching the level of the normal control group （P^0.05）. The improvement of forced vital capacity （FVC） in the Chinese medication group was superior to that in the western medication group （P〈0.05）. The indicators of tidal breathing and airway resistance were significantly improved in the Chinese medication group after treatment （P〈0. 05）. Conclusion The stage treatment with experienced Chinese herbal formulas could effectively control the symptoms and improve the lung function in children with asthma.
