细胞因子在髌下脂肪垫损伤动物模型中的变化 被引量:17

CHANGES OF CYTOKINES IN THE RABBIT MODEL OF INFRAPATELLAR FAT PAD INJURY

摘要 目的 :髌下脂肪垫损伤的病理基础是反复发作的慢性损伤以及脂肪垫夹挤和撞击引起的脂肪垫增生和炎症。本研究通过建立家兔膑下脂肪垫和滑膜损伤的模型 ,观察细胞因子IL 1 β ,IL 6和TNF的水平变化 ,以探讨它们在发病机制中的作用。方法 :应用水囊压迫兔膑下脂肪垫 ,水囊内压力保持 330~ 34 0mmHg ,配合膝关节屈伸运动 ,每天 2小时。压迫 7天后 ,于对照组与损伤组关节腔内注入 0 .5ml生理盐水进行冲洗后 ,对关节冲洗液进行放射免疫测定。结果 :损伤组IL 1 β 1 .5 3± 0 .5 1pg/ml,对照组IL 1 β 0 .6 8± 0 .2 2 pg/ml(P <0 .0 1 ) ;IL 6和TNF呈非正态分布 ,应用秩和检验 ,损伤组高于对照组 (P <0 .0 1 )。结论 :在髌下脂肪垫损伤的家兔模型中 ,关节冲洗液中所检测的IL 1 β ,IL 6和TNF等炎性细胞因子损伤组与对照组相比均显著升高 ,说明炎性细胞因子在膑下脂肪垫损伤的发病机制中起重要作用。 Objective:The pathological basis of Infrapatellar fat pad injury is repeated injury, and impingement of the infrapatellar fat pad causes fat pad proliferation and inflamation. The present study establishs the rabbit model of infrapatellar fat pad and synovial injury, observes level of cytokines IL 1β,IL 6 and TNF,and explores the pathogenic mechanisms.Methods: We used pockets of water compressing rabbit infrapatellar fat pad, the pressure in water pockets remaining 330~340mmHg, accompanied by knee joint flexion and extension 2 hours every day. Seven days later, 0.5ml of normal saline was injected into articular cavity for flushing, and the perfusate was used for radioimmunoassay of cytokines. Results: The concentration of IL 1β in the injury group (1.53±0.51 pg/ml) was siganificant higher than that of contral group (0.68±0.22 pg/ml (P<0.01); That of IL 6 and TNF was also higher in the injury group (P<0.01 Rank Sum Test, IL 6 and TNF). Conclusion. IL 1β, IL 6 and TNF measured in the injury group was much higher than the concentration of that in control group. The results suggest that inflammatory cytokines play an important role in the pathogenesis of infrapatellar fat pat injury.
