已婚女性宫颈细胞中人乳头状瘤病毒感染的基因分型研究 被引量:53

摘要 目的探讨已婚女性宫颈细胞中23种人乳头状瘤病毒(HPV)感染的基因型分布及临床意义,为宫颈癌的预防治疗及流行病调查提供参考依据。方法从南京市扬子石化集团公司2682例已婚女性职工的宫颈细胞标本中提取23种HPV-DNA,采用基因扩增结合基因芯片技术对其宫颈细胞进行23种HPV基因型别的检测,并对其受检者进行相关资料分析。结果 2682例已婚女性宫颈细胞标本中检出HPV感染者278例,HPV感染率为10.37%,238例为单一型阳性,阳性率为8.87%,其中HPV-43型54例,阳性率为2.01%,是最主要的感染型别,HPV-58型26例,阳性率为0.97%;混合型HPV感染40例,阳性率为1.49%,其中HPV-43型+X型8例、43型+X型+X型5例,两者占混合型感染的32.50%,是混合型感染的主要型别,其次是HPV-16型+X型9例及16型+X型+X型3例,占混合型感染的30.00%。结论 HPV-43、58型单一型及43型和16型的混合型是南京地区已婚女性宫颈细胞的主要基因型别,基因扩增结合基因芯片检测技术可应用于宫颈细胞标本,其特异性强、敏感性高,可检测多种HPV型别,对宫颈癌的预防治疗具有重要的意义。 OBJECTIVE To explore the genotyping of 23 genotypes of human papillomavirus (HPV) causing infections in cervical ceils of the married women and observe its clinical significance so as to provide basis for the prevention of cerical cancer and investigation of prevalent disease. METHODS Based on the original cell materials in department of infectious diseases, hospital of Yang Zi and department of pathology, third affiliated hospital of Nanjing traditional Chinese medical university, we mainly tested 23 HPV-DNA genotypes in cervical cell samples from 2682 cases of married women by means of PCR and gene-chips technique. RESULTS There were 278 positive cases in 2682 cervical cell samples of married women,the total HPV infection rate was 10.37% ,the HPV infection rate of the single type was 8.87% ; there were 54 (2.01%) cases of HPV-43 and 26 (0.97%) cases of HPV-58; there were 40 (1.49%) cases of mixed HPV infections, among which there were 8 cases of HPV43+Xorx and 5 cases of HPV43q-Xorxq-X that accounted for 32.50% and were the most predominant genotypes, followed by 9 cases of HPV16+X or 3 cases of HPV16+X+X(30.00%). CONCLUSIONS HPV-43, HPV-58, HPV43+X+ X, and HPV16+X+X are the main genotypes of the married women in Nanjing; gene amplification combined with gene-chip detection technique, with high specificity and sensitivity, can be applied for the detection of HPV genotypes in cervical cell samples, which has great significance in the prevention and treatment of cervical cancer.
