HLA-B27抗原与前葡萄膜炎的相关性及其临床意义

The association of various forms of anterior uveitis with HLA-B27 antigen and its clinical implication

目的 探讨HLA B2 7抗原与前葡萄膜炎的相关性及可能的意义。方法 用多聚酶链反应 (PCR)和微量淋巴细胞毒试验对 2 0例伴有关节强直性脊椎炎 (AS)的急性前葡萄膜炎 (AAU)、67例AAU、70例慢性前葡萄膜炎和 70 8个健康人进行HLA B2 7分型。结果 AS伴有AAU和单纯的AAU患者中HLA B2 7阳性率分别为 70 %和 49 2 5 % ,显著高于对照组 ( 2 97% ,P <0 0 0 1) ,慢性前葡萄膜炎患者中HLA B2 7的阳性率 ( 4 2 8% )与对照组无显著差别。结论 AAU与HLA B2 7有密切的相关性 ,在这些患者中进行HLA B2 7分型对评价葡萄膜炎的病因和预后有一定的价值。

ObjectiveTo investigate the association of various forms of anterior uveitis with HLA B27 antigen and its possible clinical implication.MethodsPolymerase chain reaction (PCR) or microcytotoxity assay was used for determinating HLA B27 on 20 patients with ankylosing spondylitis(AS) and acute anterior uveitis(AAU),67 patients with AAU,70 patients with chronic anterior uveits including idiopathic anterior uveitis and Fuchs syndrome,and 708 healthy individuals.ResultsDetermination of HLA B27 in 51 samples with both PCR and microcytotoxity assay revealed the same result,indicating that data from each method may be compared.A strong association between HLA B27 and AS and AAU (70%),and with AAU (49.25%) was noted compared with the controls (2.97%, P <0 001).No positive association was found between HLA B27 and chronic anterior uveitis.ConclusionAAU,especially in company with AS,is strongly associated with HLA B27 antigen.Determinating HLA B27 is helpful in the investigation of the etiology,and evaluation of prognosis of AAU.
