成人重症社区获得性肺炎的血清炎性因子变化及临床意义探讨 被引量:8

摘要 目的对成人重症社区获得性肺炎的血清炎性因子变化及临床意义进行探讨,分析血清炎性因子变化对成人重症社区获得性肺炎的影响。方法选择我院30例重症肺炎患者和30例非重症肺炎普通肺病患者,其中重症肺炎患者伴有心力衰竭症状,将其分成两组,并对全部患者的血清肿瘤坏死因子(TNF-α)、一氧化氮(NO)、白细胞介素(IL-6)、肌酸激酶心肌同工酶(CK-MB)含量水平进行测定。结果与非重症肺炎患者相比,重症肺炎患者的血清肿瘤坏死因子、一氧化氮、白细胞介素、肌酸激酶心肌同工酶的含量水平非常的高,在得到有效治疗后,重症肺炎患者的血清肿瘤坏死因子、一氧化氮、白细胞介素、肌酸激酶心肌同工酶的含量水平会有所降低,而非重症肺炎患者的血清肿瘤坏死因子、一氧化氮、白细胞介素、肌酸激酶心肌同工酶的含量水平变化的差异无显著性(P>0.05)。结论重症肺炎患者伴随心力衰竭时,患者的血清炎性因子会不断攀升,对患者造成严重的影响,如损害患者心肌、减弱患者心肌收缩能力、降低患者心脏血液的搏出量,从而引起患者心力衰竭。这可表明对于重症肺炎患者而言,进行抗感染处理,有效降低血清炎性因子的介导作用,可良好维护患者的心肌治疗。 Objective To adult severe community acquired pneumonia serum inflammatory factor and clinical significance of the changes of serum inflammatory factors, analysis of changes on adult patients with severe community-acquired pneumonia effect. Methods 30 patients in our hospital with severe pneumonia and 30 patients with non-severe pneumonia common pulmonary disease patients, including patients with severe pneumonia with heart failure symptoms, will be divided into two groups, and all 0fthe patient's serum tumor necrosis factor (TNF-c0, nitric oxide (NO), interleukin (IL-6), creatine kinase myocardial isoenzyme (CK-MB) levels were measured. ResnRs Compared with patients with severe pneumonia, severe pneumonia in patients with serum tumor necrosis factor, nitric oxide, interleukin, creatine kinase myocardial isoenzyme level is very high, in the effective treatment, patients with severe pneumonia serum tumor necrosis factor, nitric oxide, interleukin, creatine kinase myocardial isoenzyme content levels can be reduced somewhat, but not in patients with severe pneumonia serum tumor necrosis factor, nitric oxide, interleukin, creatine kinase myocardial isoenzyme level changes had no significant difference (P〉0.05). Conclusion Patients with severe pneumonia with heart failure, serum inflammatory factor can be climbed ceaselessly litre, the patients were severely affected, such as damage of myocardium in patients with decreased myocardial contractility, patients, reduced in patients of cardiac blood stroke volume, resulting in patients with heart failure, This may be indicated for patients with severe pneumonia, antibiotic treatment, effectively reduce serum proinflammatory cytokine mediated effect, can be maintained in patients with myocardial treatment.
