P^(16)蛋白和PCNA在肝细胞性肝癌中的表达及意义

Expressions and Significances of P^(16) Protein and Proliferationg Cell Nuclear Antigen in Hepatocellular Carcinoma

目的 :探讨肝细胞性肝癌 (HCC)中抑癌基因P16 蛋白及增殖细胞核抗原 (PCNA)的表达、相关性及其与肿瘤临床病理的关系。方法 :采用免疫组化SP法检测 4 5例HCC及癌旁肝组织中P16 蛋白及PCNA。结果 :①P16 蛋白及PCNA在HCC中的阳性表达率分别明显低于、高于癌旁肝组织 (P <0 .0 5)。②高分化、中分化HCC中P16 蛋白的阳性表达率明显高于低分化HCC(P <0 .0 5) ;高分化HCC中PCNA的阳性表达率明显低于中分化及低分化HCC(P <0 .0 5)。③P16 蛋白与PCNA在未转移组HCC中的阳性表达率分别明显高于、低于转移组HCC(P <0 .0 5)。④临床Ⅰ期与Ⅲ期HCC比较 ,P16 蛋白的阳性表达率及PCNA的阳性表达率均有显著性差异 (P <0 .0 1 )。⑤P16 蛋白及PCNA在HCC中的表达与肿瘤直径无关。⑥HCC中P16 蛋白与PCNA的表达间存在明显负相关关系 (r =-0 .57,P <0 .0 5)。结论 :P16 基因及PCNA的表达与HCC的增殖、分化、转移状态及临床分期有关 ,且存在明显负相关关系 ,其表达状况可以作为判断HCC恶性程度及预后的有用指标。

Objective To investigate the expression,relativity and their clinicopathological significance of P 16 protein and proliferating cell nuclear antigen (PCNA) in hepatocellular carcinoma(HCC).Methods P 16 protein and PCNA in HCC and their neighboring regions from 45 patients were studied by using immunohistochemical S P method.Results The positive expression of P 16 protein and PCNA in HCC was significantly lower or higher than in their neighboring regions.P 16 protein expression in well and moderately differetiated HCC was more obvious than in poorly differentiated one (P<0.05),while PCNA expression in highly differentiated HCC was obviously lower than in moderately and poorly differentiated HCC.The positive rate of P 16 protein and PCNA in the carcinoma with metastasis was obviously higher or lower than that in the samples without metastasis.About the positive rate of P 16 protein and PCNA,there was a statistically significant difference (P<0.05) between the cases in clinical stage Ⅰ and Ⅲ.There was no relationship between the size of tumor and the expression of P 16 gene and PCNA.In the cases of HCC investigated,there was obvious negative correlation between the expression of P 16 protein and PCNA (r=0.57,P<0.05).Conclusions The expressions of P 16 protein and PCNA are closely relating to the proliferation,differenation,metastasis and clinical stage of HCC and there is an obvious negative correlation between the two indexes.Their expression′s states can be regarded as an useful index determining the malignancy degree and prognosis of HCC.
