Correlations between blood lipids and puberty in adolescence

摘要 目的描述青春期学生血脂水平及其与性发育和性激素水平之间的关联,为制定青春期学生血脂标准和开展青春期脂代谢异常的研究提供依据。方法 2011年4-6月,随机抽取上海市2个区6所学校1 298名六至八年级学生,询问青春期发育的情况、测量身体形态并检测血脂、性激素等指标。结果影响男生血脂水平的因素主要是腰围、年龄和发生遗精,三酰甘油和高、低密度脂蛋白与腰围有关(B值分别为0.016,-0.008,0.011,P值均<0.01),总胆固醇、高密度脂蛋白与年龄有关(B值分别为-0.112,-0.046,P值均<0.01),发生遗精男生的低密度脂蛋白低于未发生遗精的男生(B=-0.163,P<0.05)。影响女生血脂水平的因素主要是腰围和发生月经初潮,三酰甘油和高、低密度脂蛋白与腰围有关(B值分别为0.134,-0.009,0.110,P值均<0.01),发生月经初潮以后女生的总胆固醇和三酰甘油低于未发生月经初潮的女生(B值分别为-0.134,-1.144,P值均<0.05)。结论青春期学生血脂水平受到年龄和青春期发育的影响,腰围对血脂的影响较大。评估青少年血脂水平时,需要将年龄和青春期发育的因素考虑进去。 Objective To study the association of puberty development with the four blood lipid components. Methods Investigation including questionnaires, physical examination and venous blood test was conducted on the randomized collected 1 298 students from 6 middle schools in Shanghai during April -June, 2011. Anthropometric indices were measured in physical examinations. Total cholesterol, low-density lipoprotein cholesterol ( LDL - C ), high-density lipoprotein cholesterol ( HDL - C ), triglyceride and sex hormone in blood samples were checked. Results It was found that lipids were significantly correlated with waist circumference, age, and occurrence of spermatorrhea among boys, which showed that the levels of triglyceride and low-density lipoprotein cholesterol were related to waist circumference, total cholesterol and high-density lipoprotein cholesterol were related to age, and the lipids levels decreased after occurrence of spermatorrhea. For girls, lipids were correlated significantly with waist circumference and occurrence of menarche, which the levels of triglyceride, high-density lipoprotein cholesterol, and low-density lipoprotein cholesterol were related to waist circumference, and the lipids levels decreased after occurrence of menarehe. In addition, it showed that central obesity was the main influencing factors to lipids levels. Conclusion Lipid levels in blood are associated with age and indices of puberty development in adolescent, among which waist circumference has the obvious influence on blood lipids. Evaluation of blood lipid level should take into account variation by age and puberty development.
