摘要
目的 研究大黄对烫伤后肝脏内肿瘤坏死因子 (TNF -α)基因表达的影响。 方法应用随机、前瞻性研究方法 ,采用大鼠烫伤模型。烫伤后 12h给予内毒素行“二次打击” ,观察大黄对肝脏内TNF -α基因表达的影响 ,以及对体内TNF -α和门、体静脉血内内毒素的清除作用。结果 正常对照组和单纯烫伤组肝脏内TNF -α没有明显表达 ,血浆和肝脏内未测到TNF -α蛋白。“二次打击”后肝内TNF -α基因表达明显增加 ,且血浆和肝组织内TNF -α浓度显著提高。大黄可明显抑制肝脏内TNF -α基因表达 ,降低血浆和肝组织内TNF -α含量。烫伤可使肠道内内毒素吸收入血 ,“二次打击”后门、体静脉血内内毒素浓度更高。大黄治疗后门、体静脉血内内毒素浓度显著下降。 结论 大黄可抑制烫伤和后继“二次打击”后肝脏内TNF -α基因表达 ,促进体内内毒素清除。
Objective To test action of rhubarb on gene expression of tumor necrosis factor-α (TNF-α) after liver scald. Methods The model rats with scald were used. Endotoxin at a dose of 20 mg/kg was continuously infused a “second hit” at 12 hours after scald. Through prospective and random way of experiment, the effect of rbubarb on gene expression of TNF-α in liver was detected. The clearing action of rhubarb on endotoxin in portal and femoral blood was also measured. Results There was little gene expression of TNF-α in normal control and scald group. Furthermore, TNF-α protein was not detected in serum and liver in those two groups. However, the “second hit' could promote gene expression of TNF-α. The concentration of TNF-α in serum and hepatic tissues were increased significantly. Rhubarb obviously inhibited gene expression of TNF-α in liver and lowered the concentration of TNF-α in serum and hepatic tissues. The burn injury damaged the gut barrier and resulted in absorption of endotoxin into the gut. The “second hit' made the concentration of endotoxin in both of the portal and femoral blood increased. Rhubarb could reduce the endotoxin level in both blood. Conclusions Rhubarb can inhibit scald and gene expression of TNF-α in liver following the second hit and promote the clearance of endotoxin.
