超声弹性成像与乳腺BI-RADIS分级联合应用于65例病灶分析 被引量:4

摘要 目的探讨超声弹性成像与乳腺图像和报告数据体系(BI-RADIS)联合应用在乳腺肿块良恶性鉴别诊断中的应用价值。方法对65例乳腺肿块进行二维超声及超声弹性成像检查,判断乳腺BI-RADIS分级与病灶弹性评分,并与病理结果进行对照分析。结果65例病灶中,良性28例,其中总分小于或等于6者27例;恶性37例,其中总分大于6者35例。用该标准进行诊断,准确度为95%(62/65),敏感度为95%(35/37),特异度为96%(27/28)。结论超声弹性评分与乳腺BI-RADIS分级之和大于6者恶性可能性增大,小于或等于6者良性可能性较大。超声弹性成像与BI-RADIS联合应用可更加客观地判断乳腺肿块的良恶性,对确定是否进行早期手术具有重要临床意义。 Objective To investigate the application significance of sonoelastography combined with breast imaging and reporting date system (BI-RADIS) in differential diagnosis of benign and malignant breast masses. Methods Two-dimensional ultrasound imaging and sonoelastography were performed on 65 breast masses. BI-RADIS grading and lesion elasticity score were determined, and they were compared with pathological results. Results Among 65 lesions, 28 were benign, including 27 with total score of 6 or lower than 6. There were 37 malignant lesions, including 35 with total score higher than 6. The accuracy, sensitivity and specificity of diagnosing by sonoelastography and BI-RADIS was 95% (62/65), 95% (35/37)and 96% (27/28), respectively. Conclusion The risk of malignant lesion is increased when the score of ultrasound elasticity score and BI-RADIS grading higher than 6. Sonoelastography combined with BI- RADIS can be used to determine the benign or malignant lesions more objectively, and it is valuable for determining surgery at early stage.
