不同穿刺方法对减轻卵巢癌患者静脉化疗疼痛的效果研究

Effects of different puncture methods on relieving chemotherapy pain of patients with ovarian cancer

目的比较3种穿刺方法对减轻静脉输注化疗药物疼痛的效果。方法临床观察240例静脉输注顺铂的患者，将其随机分为3组，每组80例，分别应用常规穿刺法、热敷静脉法和翻转针柄穿刺法固定给药，评估患者疼痛情况。结果3组穿刺方法对卵巢癌患者静脉化疗疼痛情况显示，差异有统计学意义（χ2=34．14，P〈0．01），采用翻转针柄法穿刺与其他两组相比，静脉输注化疗药物时疼痛明显减轻，差异有显著统计学意义（χ2值分别为31．85和16．63，均P〈0．01），3组疼痛程度轻度及以下发生率，翻转针柄法为57．50％，常规穿刺法组为21．25％，热敷静脉法为30．00％，差异有显著统计学意义（χ2=11．09，P〈0．01）。结论翻转针柄法是减轻静脉输注化疗药物引起疼痛的有效方法，其操作简单，效果稳定，值得推广。

Objective To compare the effects of 3 puncture methods on relieving pain caused by intravenous infusion of chemotherapeutic drug. Methods Clinical observation was made on 240 cases of patients with intravenous infusion of cisplatin. They were randomly divided into three groups with 80 cases in each, and drugs were given with conventional puncture method, hot compress vein puncture method and reversed needle handle puncture method, respectively. Then the pain of patients was determined. Results The effects of 3 puncture methods on chemotherapy pain of patients with ovarian cancer showed that the differences were statistically significant （χ2 = 34. 14, P 〈 0.01 ）. Compared with the pain with other two methods, the pain with reserved needle handle puncture method relieved markedly, and the differences were significant （χ2 value was 31.85 and 16.63, respectively, both P 〈0.01 ）. The incidence rates of mild pain or less were 57.50%, 21.25% and 30.00% in reversed needle handle puncture group, conventional puncture group and hot compress vein puncture group, respectively. There were significant differences among them （χ2 = 11.09 ,P 〈 0.01 ）. Conclusion Reversed needle handle method is an effective and simple method to relieve pain caused by intravenous infusion of chemotherapeutic drug, and its effect is stable and it is worth popularizing.
