改良B-Lynch缝合术预防剖宫产术中宫缩乏力性子宫出血的临床应用

The Clinical Applications of Modified B-Lynch Suture in Preventing Uterine Atony Bleeding for Cesarean Section

目的探讨改良B-Lynch缝合术在预防剖宫产术中宫缩乏力性子宫出血的疗效。方法选择2010年9月至2011年9月具有产后出血高危因素、并有出血倾向的产妇作为实验组,对照组为2009年9月至2010年9月具有上述情况的产妇。实验组行改良B-Lynch缝合术,对照组则常规给予药物治疗,术后或配合使用宫颈钳钳夹宫颈。结果实验组30例产妇均成功,产后出血发生率、产后出血量,血色素下降程度均低于对照组,差异有统计学意义。而子宫缩复情况,月经复潮、子宫形态均正常。结论改良B-Lynch缝合术式简单安全,止血效果好,为预防剖宫产术中宫缩乏力性子宫出血的首选而有效的方法,可在各级医院,特别是基层推广施行。

Objective To investigate the efficacy of the modified B-Lynch suture in preventing uterine atony bleeding for cesarean section.Methods The pregnant women with postpartum hemorrhage risk factors from September 2010 to September 2011 were elected as the experimental group and the control group was elected from September 2009 to September 2010.The experimental group was accepted modified B-Lynch suture and control group was given medication routinely or used cervical clamp after surgery.Results The incidence of postpartum hemorrhage,postpartum hemorrhage and the degree of hemoglobin decreased of 30 cases of the experimental group were lower than the control group,and the difference was significant.The uterine contractions,menstruation,uterine morphology were normal of the experimental group.Conclusion The modified B-Lynch suture is worthy of promotion at all hospitals especially at the grass-roots hospitals for its significant effect in preventing uterine atony bleeding for cesarean section.
