摘要
目的:研究补偿曲线对上颌全口义齿基托下支持组织应力分布的影响,为预防上颌骨组织吸收提供理论依据。方法:用三维有限元应力分析法研究不同补偿曲线时上颌全口义齿基托下支持组织应力及其分布状况。结果:前牙加载时,Ⅳ号模型唇系带切迹区压应力最大;单侧后牙加载时,Ⅲ号模型腭顶区压应力最大;双侧后牙加载时,Ⅲ号模型唇系带切迹、切牙乳突、腭顶、后牙嵴顶区压应力最大;全牙列加载时,Ⅳ号模型唇系带切迹区压应力最大。结论:在牙合力作用下,正常补偿曲线深度较异常补偿曲线深度的全口义齿模型,义齿基托下骨组织压应力值较小。义齿修复前应对下颌天然牙进行调牙合、重建正常的牙合曲线以利于全口义齿基托下骨组织的保健。
Objective: To study the stress distribution on the supporting tissue of the maxillary complete denture with different compensate curve. Methods: The 3D FEM was used to study the stress distribution on the supporting tissue of the maxillary complete denture with different compensate curve. Results: Loading on anterior teeth, the stress concentration was obvious in the labial frenum notch zone ofedentulous ridge in the fourth model. Under unilateral posterior teeth loading conditions, the stress of the third model was highest in the area of anterior palatum. Under bilateral posterior teeth loading conditions, the stress of labia frenum notch zone, incisive papilla zone, middle palatum and posterior teeth crest were highest in the third model. Loading on all the teeth, the stress was highest in the labial frenum notch zone in the fourth model. Conclusion: Under occlusal force, the stress of the supporting tissue of the maxillary complete denture with normal compensate curve was lower than that of the complete denture with abnormal compensate curve. Therefore, to protect the health of alveolar bone, it is necessary tocorrect the abnormal compensate curve of the mandible teeth before the fabrication of a maxillary denture.
