乙肝疫苗与乙肝免疫球蛋白联合阻断乙肝母婴传播的免疫效果观察 被引量:9

Observation on the immunity effect of hepatitis B vaccine and HBIG(hepatitis B immunoglobulin) in interruption of hepatitis B transmission from mother to infant

摘要 目的:观察乙肝疫苗与乙肝免疫球蛋白联合阻断乙肝母婴传播的效果,寻找乙肝疫苗母婴阻断的最佳免疫策略。方法:母亲HBsAg阳性新生儿在出生24 h内接种不同剂量的HBIG和乙肝疫苗,分别检测全程免疫接种后1个月、6个月儿童抗体滴度。结果:⑴共调查463例新生儿乙肝表面抗原、抗体滴度,抗-HBs阳转率为96.49%,7月龄与12月龄儿童抗-HBs(>10 IU/ml),比较差别无统计学意义(P>0.01)。⑵母亲双阳与母亲单阳的儿童,抗-HBs滴度差别有统计学意义(P<0.01)。⑶接种HBIG100 IU与200 IU的12月龄儿童抗-HBs阳转率,差别无统计学意义(P>0.01)。结论:乙肝疫苗加HBIG联合接种具有良好的免疫效果,尤其在母亲为HBsAg和/或HBeAg阳性的新生儿中与乙肝免疫球蛋白联用可达到较好的母婴阻断效果。 Objective:To observe the effect of combined application of hepatitis B vaccine and HBIG in interrupting mother-infant transmission of hepatitis B and find out the best immunity strategy to interrupt hepatitis B mother-infant transmission.Methods: The children′s antibody titer was tested in lmonth and 6months after their inoculation of different doses of HBIG and hepatitis B vaccine within 24hours after their birth.Results: 1.The anti-HBs positive conversion rate of total 463 neonates′ HBsAg and antibody titer was 96.49%.The difference between 7 m neonates and 12 m neonates(10 IU/ml) had no statistic significance(P0.01).2.For the neonates with mother HBsAg + and HBeAg + and the neonates with mother HBsAg +,the anti-HBs titer difference had statistic significance(P0.01).3.For the positive conversion rate of 12 m children inoculated with HBIG 100 IU and 200 IU,the difference had no statistic significance(P0.01).Conclusion: Combined immunity of hepatitis B vaccine and HBIG has good immunity effect,especially for the neonates with mother HBsAg+ and/or HBeAg +,the combination can realize the good mother-infant interruption protection.
