头高足低位在新生儿足跟采血中的应用

Application of the posture of head raised and feet lowered in blood collections from the neonatal heels

目的探讨头高足低位在新生儿足跟采血中的应用效果。方法选择本院2009年3月～2010年4月实施足底采血进行先天性甲状腺功能低下和苯丙酮尿症筛查的新生儿120例,将新生儿随机分为观察组与对照组,每组各60例,观察组新生儿沐浴后采取头高足低位采血,对照组新生儿沐浴后采取传统的平卧位采血。比较两组新生儿采血时间、一次采血成功率及溢奶发生率情况。结果观察组新生儿一次采血成功率明显高于对照组,采血时间明显短于对照组,溢奶发生率明显低于对照组,两组比较,差异具有统计学意义(均P<0.05)。结论头高足低位采血具有采血时间短、溢奶发生率低、一次采血成功率及血片质量高等优点,值得临床推广应用。

Objective To investigate the effect of applying the posture of head raised and feet lowered during blood collections from the neonatal heels.Methods 120 neonates receiving blood collection from the neonatal heels for screening for congenital hypothyoidism and phenylketonuria were evenly divided into the observation and control groups.The former group received blood collection with head raised and fee lowered in blood collections from the neonatal heels and the later did with routine prostration after their bathing.The two groups were compared in terms of time for blood collection from infants,success rate for one-time blood collection and incidence of spilled milk.The success rate of one-time blood collection in the observation group was higher than that of the control group.Results The success rate for one-time blood collection was significantly higher than that of our country（P 〈0.05）,time for collecting blood was significantly shorter than that of the control group（P 〈0.05）,the occurrence rate of spilled milk was significantly lower than that of the control group（P 〈0.05）.Conclusions Blood collection by position of head raised and feet lowered may be advantageous in time for blood collecting,lower rate of spilled milk,higher success rate for one-time blooding collection and high quality of collected blood.So it deserves clinical application.
