内镜经鼻孔-蝶窦入路手术治疗垂体瘤71例

Clinical analysis of pituitary tumors wia endoscopic endonasal for hypophysoma

目的探索内镜经鼻蝶手术中的临床意义和应用前景。方法对71例垂体腺瘤患者行内镜经鼻蝶入路手术,在手术之前均作影像学及血液的检查,观察术后结果。结果共有51例的垂体瘤全部切除,仅有3例需要进行二次切除,还有2例大部分切除,术后并无死亡者。47例术后康复至正常水平,34例在手术之后6个月内激素明显增高,随之进行二次手术全部切除。结论内镜经鼻蝶入路手术治疗垂体瘤手术中的运用,创伤很小,操作简单安全、术后患者反应良好,能有效缩短住院时间。

Objective To explore the clinical significance and future perspective of neuroendoscopy in ranssphenoidal surgery. Methods 71 patients with hypophysoma were treated by endoscopic endonasal transsphenoidal surgery.All the patients received neuroimaging（MR and CT）and endocrinological examination before surgery, observed the postoperative. Results The tumor removal was total in 51 cases,only 3 cases needed to be done second times operation. two patients had the most resection.there was no patients died after surgery,the postoperative rehabilitation after surgery to normal levels of 47 patients.the hormone level of 34 patients in 6 months after surgery was significantly higher , need to be done second times operation surgery. Conclusion The clinical effect of endoscopic transsphenoidal surgery for pituitary tumors is good , the trauma is very small,clear exposure of operation field,relatively complete resection of tumor,simple operation,safety,mild response and shorter hostitalization time.
