摘要
目的探讨老年胃十二指肠急性穿孔的病因诊断与治疗方法,提高救治水平。方法对笔者所在医院2002年3月～2012年3月收治的67例60岁以上的老年胃十二指肠急性穿孔患者的临床资料进行回顾性分析。结果保守治疗5例,单纯穿孔修补术52例,胃大部切除术10例,死亡7例。结论老年胃十二指肠穿孔,病史长,溃疡瘢痕大,不易愈合,胃癌穿孔几率高,并发症多,一经确诊应及早手术治疗。
Objective To discuss the etiological diagnosis and treatment methods of the acute perforation of stomach and duodenum in elderly patients,improving the treatment level. Methods The clinical data in March 2002 to March 2012 of 67 cases who were over 60 years old with acute perforation of stomach and duodenum were retrospectively analyzed. Results Conservative treatment in 5 cases,simple perforation repair in 52 cases, subtotal gastrectomy in 10 cases, 7 cases of death. Conclusion The acute perforation of stomach and duodenum in elderly patients has the following characteristics, long medical history,big ulcer scar,not easy to heal,high risk of stomach perforation,and more complications.Once diagnosed, early operation treatment is necessary.
