上肢有创血压与无创血压的差异分析

Discrepancy analysis on intra-arterial blood pressure and non-invasive blood pressure of upper limb

目的研究桡动脉有创动脉压与肱动脉无创动脉压之间的差异及其影响因素。方法对入住ICU的60例患者进行IABP与NBP的同步测量。结果收缩压均值,右侧IABP最高,左侧NBP最低。NBP与IABP收缩压测量值之间分组比较,PPI<0.45时NBP9.38%>IABP;0.45≤PPI≤0.55时NBP42.59%>IABP;PPI>0.55时NBP58.82%>IABP。结论在临床实践中,NBP与IABP的实际差异与理论相比有所不同。监测血压时应首选右臂,应用IABP时使用方波试验来判断IABP值的准确性;使用NBP时应对患者的血管硬化程度进行评估。

Objective To investigate discrepancy between intra-arterial blood pressure of radial artery and non-invasive blood pressure of brachial artery,and its influence factor. Methods IABP with NBP simultaneous monitoring of sixty patients who admission to the ICU treatment. Results Systolic blood pressure mean,the highest of the right side of IABP,the minimum of the left side of NBP.Grouping compare NBP with IABP,systolic blood pressure between the measured values,PPI 0.45,NBP 9.38% greater than the IABP;0.45 ≤ PPI ≤ 0.55,NBP 42.59% greater than the IABP;PPI 0.55,NBP 58.82% greater than the IABP. Conclusion In clinical practice,compared with the theoretical,there are discrepancy on the differences of NBP with IABP.Monitoring of blood pressure should be the preferred right arm,square wave test to determine the accuracy of the IABP;response to the patient＇s degree of hardening of the arteries to assess before using NBP.
